针刺内关穴对心肌缺血／再灌注损伤大鼠脊髓背角和背根神经节内5-羟色胺表达的影响

摘　　要：目的探讨针刺内关穴对缺血／再灌注（I／R）损伤大鼠心肌的保护作用及其与脊髓背角和背根神经节（DRG）内5-羟色胺（5-HT）的关系。方法将40只大鼠随机分为正常对照组、假手术组、模型组、针刺组4组，每组10只。结扎大鼠左冠状动脉前降支造成心肌I／R损伤模型，假手术组只穿线不结扎；针刺组在针刺大鼠内关穴5d后再行I／R。采用分光光度计测定血清缺血修饰白蛋白（IMA）含量，用免疫组化法测定脊髓背角和DRG内5-HT阳性表达量。结果模型组大鼠心肌I／R损伤后心电图STⅡ电位值、血清IMA含量、脊髓背角和DRG内5-HT阳性表达量均显著高于正常对照组和假手术组，而针刺组上述指标则较模型组显著降低（P〈O．05或P〈0．01）。结论针刺内关穴能减少血清IMA含量，降低心电图STⅡ电位值，对缺血心肌具有保护作用，其机制可能与针刺通过干预脊髓背角和DRG内5-HT的表达，抑制缺血伤害性刺激的传导，减轻心肌应激性损伤有关。Objective To observe the protective effect of acupuncture at Neiguan point （内关穴） on myoeardia in rats with myocardial isehemia/reperfusion （I/R） injury and explore the relationship between the effect and the content of 5-hydroxytryptamine （5-HT） in the dorsal spinal cord and dorsal root ganglion （DRG）. Methods Forty Sprague-Dawley （SD） rats were randomly divided into four groups： normal control, sham operated, model and acupuncture groups （each n= 10）. The rat models of I/R were established by ligating the left anterior descending coronary artery. The artery was not ligated, but a thread was passed around it in the sham operated group. The rats took acupuncture at Neiguan point everyday for 5 days, and then I/R was performed on them in the acupuncture group. Speetrophotometry was used to detect the content of ischemia modified albumin （IMA） in serum, while immunohistochemical method was applied to detect the expression of 5-HT in the dorsal spinal cord and the DRG. Results The value of ST I in electrocardiogram （ECG）, the content of IMA in serum and expression of 5-HT in dorsal spinal cord and DRG in model group were all significantly higher than those in the control and sham operated groups. But the indexes mentioned above of acupuncture group were significantly lower than those in the model group （P〈0.05 or P〈0.01）. Conclusion Aeupuncture at Neiguan point can decrease the content of IMA and degrade the value of ST I in ECG, so it may protect myocardia in rats with I/R. The mechanism is probably related to the regulation of the expression of 5-HT in dorsal spinal cord and DRG, and inhibition of transduction of ischemia noxious stimulation, thus the myocardial irritability injury is reduced.
