摘 要:目的通过建立急性大面积肺血栓塞症(PTE)心肌受损的家兔模型,研究吸入一氧化氮对大面积PTE家兔血中心肌肌钙蛋白I(CTnI)浓度的影响。方法日本大白兔30只,分为实验组10只,只对家兔进行大面积PTE心肌受损的模型实验;对照组10只,常规溶栓治疗;治疗组10只,常规溶栓治疗+吸入NO,NO吸入浓度为40×10-6g/L,实验组、对照组、治疗组于栓塞成功后每4小时抽血1次共48h。结果家兔大面积肺栓塞后8hCTnI全部阳性,其峰浓度出现所需时间(19±5)h,CTnI阳性持续时间(39±5)h,其CTnI峰浓度值为(0.42±0.12)μg/L,其CTnI峰浓度出现所需时间和阳性持续时间对照组较实验组明显缩短[(15.1±3.2)hvs(18.8±4.5)h,(P<0.05);(34.1±3.5)hvs(38.6±5.2)h,(P<0.05)],治疗组较对照组亦明显缩短[(12.4±2.4)hvs(15.1±3.2)h,(P<0.05);(31.0±2.2)hvs(34.1±3.5)h,(P<0.05)],CTnI峰浓度对照组较实验组明显降低[(0.31±0.10)μg/Lvs(0.42±0.12)μg/L,P<0.05)],治疗组较对照组亦明显降低[(0.21±0.06)μg/Lvs(0.31±0.10)μg/L,(P(0.05)]。结论吸入NO能明显保护大面积PTE家兔心肌受损。Objective To observe the effect of inhalation of nitric oxide on the cardiac troponin I (CTnI) of rabbits with acute massive pulmonary embolism (PTE) by making the animal model of myoeardium injure of the rabbits with acute massive pulmonary embolism. Methods 30 Japanese white rabbits were randomly divided into experiment group, control group ( routine therapy ) , treatment group ( routine therapy combined with nitrin oxide inhalation of 40 × 10^-6g/L). The concentration of CTnI was detected at an interval of 4h for 48h after animals models had been successfully made. Results The cardiac troponin I of rabbits with acute massive pulmonary embolism became positive 8h after PTE, and the time of peak was (19 ± 5 )h,the time of positive CTnI was (39 ±5 )h, and its peak concentration was (0. 42 ± 0. 12 ) μLg/L, and the time of its peak and positive CTnI in control group was significantly shorter than that of experiment group [ ( 15.1 ± 3.2 ) h vs ( 18.8 ± 4.5 ) h, P 〈 0.05 ; ( 34.1 ±3.5 ) h vs ( 38.6 ± 5.2 ) h, P 〈 0.05 ] , and which in treatment group was significantly shorter than that of control group[ (12.4±2.4)hvs (15.1 ±3.2)h,P 〈0.05; (31.0 ±2.2)hvs (34.1 ±3.5)h,P 〈0.05]. The peak concentration of CTnI in control group was significantly lower than that of experiment group [ (0.31 ± 0. 10) μg/L vs (0.42 ± 0.12 )μg/L, P 〈 0.05 ] , which in treatment group was significantly lower than that of control group [ (0.21 ± 0.06)μg/L vs (0. 31 ± 0. 10)μg/L, P 〈 0. 05 ]. Conclusion The nitric oxido inhalation can obviously relieve the myocardial damage in rabbits with acute massive pulmonary embolism.
