床旁经腹部超声引导在清宫术及取环术中的应用

摘　　要：目的探讨超声引导下各种复杂清宫术、取环术的临床应用价值。方法在超声实时监视下,对207例各种原因引起的宫内残留物实施清宫术及取环术。结果207例患者中,除一例因节育器嵌入子宫肌层斜穿子宫右后壁接近浆膜层无法取出外,其余均一次成功,术后无再次残留、漏吸及大出血、感染、子宫穿孔、肠管及膀胱损伤等并发症。结论超声引导下进行复杂清宫术及取环术,避免了传统手术的盲目性、危险性,提高了手术的成功率与安全性,缩短了手术时间,减少了术后并发症的发生,具有重要的推广和应用价值。Objective To explore the role of clinical application of various kinds of uterine curetrage and removing intrauterine contraceptive device by ultrasonic guidance. Method Under the realtime ultrasound, uterine curettage and removing intrauterine contraceptive device were performed for 207 cases with intrauterine remainder caused by various reasons. Results The surgery was successfully performed in all 207 cases except 1 case duo to the intrauterine contraceptive device inserted into the myometrium and through the right posterior wall of the uterus near the serosal layer. There were no complications such as residual,leaky, hemorrhoea,infection, uterine perforation, intestinal cannal and bladder injury. Conclusion The operation can avoid the blindness and risk of traditional operation and increase the achievement ratio and safety, shorten operation time, decrease the postoperative complications,and is of important value in generalization and application.
