特发性血小板减少性紫癜Fas-FasL表达与中医辨证分型的关系

摘　　要：目的:探讨特发性血小板减少性紫癜(ITP)患者外周血T淋巴细胞亚群(简称T亚群)表面凋亡调控因子Fas/FasL表达及其与中医辨证分型的关系.方法:按中医辨证分型将80例ITP患者分为4组,每组20例.以流式细胞术检测ITP患者外周血T亚群表面凋亡调控因子Fas/FasL的表达率并与对照组(正常健康志愿者20例)相同指标进行比较.结果:CD4+及CD8+外周血T亚群表面凋亡调控因子Fas/Fasl表达率ITP瘀血阻络组、血热妄行组治疗前与对照组相比较显著增加(P<0.05),血热妄行组治疗后显著下降,瘀血阻络组略有上升,但差异无显著性.CD4+及CD8+外周血T亚群表面凋亡调控因子Fas/Fasl表达率ITP阴虚火旺组、气虚不摄组治疗前与对照组相比较有所增加,治疗后略有上升,但差异均无显著性.结论:ITP患者中医辨证分型与Fas/FasL表达存在相关性.ITP患者较高的T亚群表面凋亡调控因子Fas,Fasl值,提示好的预后.AIM： To explore the expression of apoptosis associated proteins Fas/FasL of T lymphocytes in patients with idiopathic thromboeytopenic purpura（ITP） and to study its correlation with Chinese medical syndromes. METHODS： Eighty eases of ITP patients were divided into 4 groups based on Chinese medical differentiation-classification and 20 healthy volunteers were employed as normal control group. The expression rate of membrane Fas/FasL of T lymphocytes was determined by flow cytometry. RESULTS： Compared with that in healthy control group, the expression rate of Fas/FasL on CD 4^＋and CD8^＋ T cells membrane significantly increased in groups of blood-heat （Xuere-wangxing） and ecchymosis-stay（Yuxue-zuluo） （ P 〈 0.05 ）. Compared with that before treatment, the expression rate of Fas/FasL on CD4^＋ and CD8 ^＋ T cells membrane decreased in group of blood-heat after treatment, while no statistical significance was observed in group of ecchymosis-stay. Compared with that in healthy control group, the expression rate of Fas/FasL on CD4^＋ and CD8^＋ T cells membrane in groups of yin-deficiency fire-energy and air-deficiency slightly increased, with no statistical significance. CONCLUSION： Apoptotic speed of T lymphocytes in ITP patients of blood-heat （Xuere-wangxing） and ecchymosis-stay （Yuxue-zuluo） accelerates. Hormone therapy may interfere Fas/ FasL signal transduction pathway of apoptosis. Chinese medical syndromes of ITP patients is correlated with Fas/Fasl on CD4^＋ and CD8^＋ T cells membrane.
