摘 要:目的:探讨超高速CT电影运动实验对诊断冠心病的价值.方法:对18例冠状动脉粥样硬化型心脏病(冠心病)和22例非冠心病例(正常组)的超高速CT运动试验心室功能和室壁运动与冠状动脉造影(金标准)结果进行对比分析.结果:若以①运动高峰时左室射血指数下降,②左室壁局部运动异常反应,③右室壁局部运动异常反应作为诊断冠心病的指征,其准确性分别为88%(使用指征①),90%(使用指征②),93%(使用指征①+②),和95%(使用指征①+②+③).右室壁的运动异常对诊断冠心病的敏感性为60%,特异性为78%.结论:超高速CT电影运动试验对诊断冠心病有重要价值.Aim: To measure the response of left and right ventricular ejection fraction and wall motion to exercise using ultrafast computed tomography (UFCT). Methods: Coronary artery angiography and UFCT exercise cine studies were performed for the evaluation of chest pain in all 40 cases, including 18 patients with coronary artery disease (CAD) and 22 patients with normal coronary artery documented by angiography. Results: The accuracy of UFCT in identifying patients with any CAD, was 88% using LVEF, 90% using LVWMA, 93% using both LVEF and LVWMA, and 95% using a combination of LVEF,LVWMA and RVWMA. The sensitivity of RVWMA alone in detecting right coronary artery disease (RCAD) was 6/10 (60%) and the specificity was 7/9 (78%). Conclusion: Our study suggests that exercise UFCT appears to be a useful tool for the detection of CAD in patients with chest pain.
