脑卒中合并肺部感染俯卧位与侧卧位振动排痰效果对比研究

摘　　要：目的探讨脑卒中并发肺部感染患者进行振动排痰机排痰时采取侧卧位和俯卧位哪种效果更佳。方法将50例脑卒中并发肺部感染且无振动排痰禁忌的患者随机分为俯卧位组及侧卧位组,观察两组的排痰效果。结果两组振动排痰后血氧饱和度均显著提高(P<0.001),对血压无明显影响。俯卧位组较侧卧位组排痰量大(P<0.001),啰音更快消失(P<0.001),病程缩短(P<0.001)。结论侧卧位及俯卧位振动排痰均有效,俯卧位排痰效果更佳。Objective To study the better position on vibration sputum elimination for stroke patients complicating pulmonary infection. Method 50 patients with stroke and pulmonary infection without contraindication were randomly divided into two groups： prone position group and lateral position group. The effect of sputum elimination was observed and compared. Result The blood oxygen saturation increased after vibration sputum elimination in both groups （P〈0. 001）. There was no significant influence for the blood pressure. Compared to the lateral position group, prone position group showed larger volume of sputum elimination （P〈0. 001）, quicker rales disappearance （P〈0. 001） and shorten of the disease duration （P〈0. 001）. Conclusion Both lateral position and prone position are effective in vibration sputum elimination, Prone position is better
