摘 要:目的探讨尿毒症继发性甲状旁腺功能亢进症(SHPT)患者外周血单个核细胞(PBMC)转录因子GATA3 mRNA和血清γ干扰素(γ-IFN)、白细胞介素4(IL-4)表达水平变化及意义。方法30例尿毒症(CKD5期)SHPT[全段甲状旁腺激素(iPTH)≥300pg/ml]和20例尿毒症非SHPT(iPTH≤150pg/ml)患者,分离PBMCs,RT-PCR半定量法检测GATA3表达水平,ELISA法测γ-IFN、IL-4细胞因子,分析两组间测值的差异及相关性。结果SHPT组GATA3[(1.405±0.493)vs.(0.412±0.093)]和IL-4(65.582±19.043)pg/mlvs.(17.983±4.067)pg/ml]表达水平显著高于非SHPT组,γ-IFN显著低于非SHPT组[(69.842±9.751)pg/mlvs.(79.847±16.991)pg/ml],均P<0.01;GATA3与iPTH及IL-4呈正相关性。结论尿毒症SHPT患者存在免疫功能紊乱,GATA3可能参与其发病机制。Objective To explore the possible mechanism and its clinical significance of the changes of interferon factop-γ (γ-IFN)/interleukin 4 (IL-4) in ureamic patients with secondary hyperparathyroidism (SHPT). Me/hods Peripheral blood mononuclear cells (PBMCs) were separated from 30 uraemic patients with SHPT (iPTH〉300 pg/ml) (group SHPT) and 20 uraemic patients(iPTH〈150 pg/ml (group non-SHPT). The transcription level of GATAS gene was detected by semiquantitative reverse transcriptase-PCR and that of γ-IFN/IL-4 by ELISA. The difference between two groups and correlation of those were analyzed. Results The levels of GATA3 and IL-4 were significantly higher in group SHPT than those in group non-SHPT [(1. 405±0. 493)vs. (0. 412±0. 093) and (65. 582 ± 19. 043) pg/ml vs. (17. 983 ± 4. 067) pg/ml], but the γ-IFN was lower in group SHPT than that in group non-SHPT [(69. 842±9. 751) pg/ml vs. (79. 847±16. 991) pg/ml] (P〈0.01), The level of GATA3 was positively correlated to that of iPTH and IL-4. Conclusion There is an immunodisturbance in uraemic SHPT patients and GATA3 may play an important role in the pathogenesis of SHPT.
