摘 要:目的探讨2型糖尿病(T2DM)患者及大血管病变患者外周血CD4+CD28-T细胞及其肿瘤坏死因子(TNF)-α、干扰素(IFN)-γ的变化及其意义。方法应用免疫荧光、流式细胞术及ELISA法检测36例初诊单纯T2DM(A组)、30例T2DM合并大血管病变(B组)和28例正常人(C组)的外周血CD4+CD28-T细胞亚群及血清IFN-γ、TNF-α的水平。结果B组CD4+CD28-T细胞和血清TNF-α、IFN-γ水平均高于A组(P<0.05),且两组均高于C组(P<0.05)。结论自身反应性CD4+CD28-T细胞可通过高分泌IFN-γ等细胞因子参与T2DM及大血管病变发生和发展。Objective To investigate the changes and significance of CD4^+ CD28^- T lymphocyte subsets and serum tumor necrosis factor (TNF)-α,interferon factor(IFN)-γ in type 2 diabetic(T2DM) patients with maerovascular complications. Methods CD4^+ CD28^- T lymphocyte subsets of peripheral blood mononuclear cells (PBMCS) and serum TNF-α, IFN-γ level were analyzed by immunofluorescence, flow cytornetry and ELISA in 36 T2DM (group A), 30 T2DM with macrovascular complications(group B) and 28 healthy controls (group C). Results The percentage of CD4^+ CD28^- T cells and the levels of TNF-α and IFN-γ were higher in group B than those in group A (P〈0. 05) ,which were significantly lower in group C than those in groups of B and A (P〈0. 05). Conclusion Autoimmune CD4^+ CD28^- T lymphoeytes may participate in the pathogenesis and progression of T2DM and its macrovascular disease bv over secretion of IFN-γ And IFN-γ.
