摘 要:目的探讨股部小切口下微创瓣膜修复结合血管内激光或电凝治疗原发性下肢深静脉瓣膜功能不全的新方法。方法原发性下肢深静脉瓣膜功能不全336例,均取股部小切口,行第一对股浅静脉瓣膜外修复;其中276例以810nm的半导体激光对迂曲扩张的大隐静脉进行血管内光凝术;另60例选用英国产UMD3000T电刀,由套管送入钢丝,对明显迂曲扩张且较粗的浅静脉,分段电凝。结果术后当天患者既可下床,无淋巴漏、皮下血肿或下肢水肿等并发症。术后1、2、3、4和5年随访,临床治愈率分别为97.65%,93.71%,92.59%,91.05%和89.71%。结论联合瓣膜修复结合血管内激光光凝或电凝,是治疗原发性下肢深静脉瓣膜功能不全较好的方法。该方法操作简单、创伤小、恢复快、肢体美观、近期和远期疗效果较好,值得进一步推广应用。Objective To investigate the new method of valve reparation combined with intravenous laser photocoagulation to treat primary deep venous valve dysfunction of lower limbs. Methods To retrorespectively analyze data about 336 patients with primary deep venous valve dysfunction, who were performed reparation of the first pair of valves in femoral superficial vein and intravenous laser coagulation in the varicose veins. Results At the same day on the operation, the patients could get off bed without any complication. During 1 to 8 years follow-up, the clinical cure rates of first 5 years post operation were 97. 65% ,93.71% ,92. 59% ,91.05% and 89. 71%, respectively. Conclusions The valve repair operation combined with laser photocoagulation is one kind of effective, convenient, minitraumatic,aesthetic and quick restorative therapeutic method to treat patients with primary deep venous valve. It is deserved to be used widely in the future.
