摘　　要：目的探讨消瘀化痰饮对非酒精性脂肪肝患者胰岛素抵抗的影响及其作用机制。方法选择非酒精性脂肪肝患者105例,随机分为2组,治疗组65例予消瘀化痰饮,每日1剂,水煎服;对照组予复方氨酸胆碱片3片,每日3次口服,2组均30 d为1个疗程,治疗2个疗程后检测2组血清丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、总胆固醇(TC)、甘油三酯(TG)、空腹血糖(FPG)、空腹胰岛素(FINS)的含量,计算胰岛素抵抗指数(HOMA-IR),并通过B超观察肝组织影像学的变化。结果消瘀化痰饮能明显降低患者ALT、AST、TC、TG及FPG及、FINS、HOMA-IR,改善肝组织的病变程度,疗效优于对照组。结论消瘀化痰饮可通过改善胰岛素抵抗、增强肝细胞对胰岛素的敏感性,以及调节血脂、血糖代谢,达到治疗非酒精性脂肪肝的作用。Objective To explore the effect and mechanism of Xiaoyu Huatan Yin on insulin resistance of nonalcoholic fatty liver disease （NAFLD）. Methods 105 patients with NAFLD were randomly divided into two groups： the treatment group and the control group. The patients in treatment group （ n = 65 ） were treated by Xiaoyu Huatan Yin. While patients in control group were treated by routine therapy. The course was 30days in two groups. After two courses,the contents of ALT, AST, TCH, TG, FBG and INS in serum were measured respectively. HOMA - IR was calculated and the imaging change of the hepatic tissue was observed by type - B ultrasound. Result Xiaoyu Huatan Yin markedly reduced HOMA - IR and the contents of ALT, AST, TCH, TG, FBG and INS in serum and improved the state of the hepatic tissue. In addition, the therapeutic effect in the treatment group was better than that in the control group. Conclusion Xiaoyu Huatan Yin can effectively control fatty liver by improving insulin resistance, increase the sensibility of the hepatic cells on insulin and adjust the levels of blood lipid and blood glucose.
