摘 要:目的研究2型糖尿病患者β3肾上腺能受体基因型及其与胰岛素抵抗的相关性。方法选取151例中老年2型糖尿病患者,80例健康体检成年人作为对照。比较糖尿病患者与非糖尿病患者之间的各种生化指标及基因型的差异,胰岛素抵抗患者和非胰岛素抵抗患者各种生化指标及基因型的差异。结果2型糖尿病患者较年龄匹配的非糖尿患者群BMI、SBP、TG、TC、LDL-C、FPG、HbA1c明显升高,而HDL-C显著降低(P<0.05,P<0.01);中老年2型糖尿病组β3AR64Arg等位基因频率为:17%,β3ARTrp/Arg等位基因频率为:32.5%,Arg/Arg等位基因频率为:0.6%;对照组β3AR64Arg等位基因频率为:15%,β3ARTrp/Arg等位基因频率为:30%,无Arg/Arg基因型,两组间差异无统计学意义(P>0.05)。胰岛素相对抵抗组BMI、DBP、WHR、三酰甘油、FPG、FINS比明胰岛素相对敏感组显升高(P<0.05,P<0.01);胰岛素相对抵抗组Trp/Arg基因型频率为45.6%,β3AR64Arg等位基因频率为23.5%;胰岛素相对敏感组Trp/Arg基因型频率为26.9%,β3AR64Arg等位基因频率为13.5%;两组间差异有统计学意义(P<0.05)。结论2型糖尿病患者β3肾上腺能受体基因型与发病无关,但是β3肾上腺能受体基因型与胰岛素抵抗的密切相关。Objective To investigate the β3-adrenergic receptor genotypes in 2 diabetes patients, and to explore their roles in the β3-adrenergic receptor genotypes and Insulin resistance in 2 diabetes patients. Methods We compared with differences of the biochemical indicators and genotypes in 151 cases 2 diabetes patient and 80 cases healthy controls. At the same time, Comparison differences of the biochemical indicators and the genotypes in insulin resistance and insulin-sensitive patients. Results The BM1, SBP,TG, TC, LDL-C, FPG and HbAlc Were higher in 151 case middle and old ages 2 diabetes patient than 80 cases healthy controls, but the HDL-C was significantly Low( P 〈 0. 05, P 〈 0.01 ). Group 2 diabetes, the β3 AR 64Arg allele frequencies : 17%, β3AR Trp/Arg allele frequency:32. 5% ,Arg/Arg allele frequency:0. 6% ; pairs control group β3AR 64Arg allele frequencies: 15%, β3AR Trp/Arg allele frequencies: 30%, no Arg/Arg genotypes. The β3AR allele gene frequencies was no significant differences between the two groups( P 〉 0. 05 ). The BMI, DBP, WHR, TG, FPG, FINS were higher in insulin resistance group than that in insulin-sensitive group ( P 〈 0. 05, P 〈 0. 01 ). Insulin resistance group Trp/Arg genotypes relative frequency of 45. 6%, β3AR 64Arg allele frequency relative to 23.5% ; insulin sensitivity Group Trp/Arg genotypes frequencies for 26. 9% , β3AR 64Arg allele frequencies for 13.5% ; There are obvious differences between the two groups (P 〈 0. 05). Conclusion 2 diabetes in patients can be identified genotypes with the β3-adrenergic receptor genotypes unrelated, but can be identified β3-adrenergic receptor genotypes closely associated with insulin resistance.
