综合性心理干预对狼疮性肾炎患者负性情绪及生存质量的影响

摘　　要：目的探讨综合性心理干预对狼疮性肾炎患者负性情绪及生存质量的影响。方法将60例狼疮性肾炎患者随机分为对照组(n=30)和干预组(n=30),对照组给常规治疗及护理;干预组在与对照组同样治疗护理的基础上,给为期8周的综合性心理干预。于入组时和8周末采用汉密尔顿抑郁量表(HRSD),汉密尔顿焦虑量表(HAMA)为测评工具,测评干预前后两组患者的负性情绪,并采用世界卫生组织生存质量测定量表简表(WHOQOL-BREF)测评干预组患者干预前后的生存质量。结果入组时两组的患者HAMA、HRSD评分比较差异无统计学意义(P>0.05);干预治疗后干预组较对照组患者的HA-MA、HRSD评分均显著降低,差异有统计学意义(P<0.05);干预组干预治疗前后评分比较(HAMA:t=4.29,P<0.01;HRSD:t=5.43,P<0.01)差异有统计学意义;干预组干预后生存质量总分、生理领域、心理领域等得分较对照组差异有统计学意义(P<0.05)。结论综合性心理干预能够改善狼疮性肾炎患者负性情绪,提高其生存质量。Objective To investigate the Effect of comprehensiveness mental intervention on the negative mood and the quality of life of patients with lupus nephritis. Methods 60 patients with lupus nephritis were randomly divided into the study group （ n = 30 ） and the control group （ n = 30 ）. The two groups had the same clinical routine treatments and nursing for 8 weeks as a course of treatment, and comprehensiveness mental intervention was added to the study group. The scores of Hamilton depression rating scale（HRSD） and Hamilton Anxiety Scale（HAMA） on both groups were evaluated before and after the treatment. The World Health Organization quality of life questionnaire （WHOQOL-BREF） were used and investigated on their quality of life. Results There was no significant difference on the scores of HRSD and HAMA between the two groups before therapy（P 〉0. 05 ）. The scores on HRSD and HAMA of the study group after the treatment notably dropped compared to the control group, the difference was statistical significance （P 〉 0. 05 ）, and they also was statistical significance compared to the scores before comprehensiveness mental intervention（ HAMA ： t = 4. 29, P 〈 0.01 ; HRSD ： t = 5.43 ,P 〈 0. 01 ）. The difference of scores on overall ,physiological functions and psychological aspect of WHO- QOL-BREF before and after the treatment was statistical significance. Conclusion Comprehensiveness mental intervention plays an important role in the treatment of patient with lupus nephritis to improve their negative mood and quality of life.
