摘  要:目的探讨肾脏CT灌注参数在60岁以上健康老年人中的变化。方法30例正常人按年龄分两组:I组40~59岁,15例,II组60~79岁,15例,全部采用Siemens16层螺旋CT行双侧肾脏灌注扫描。采集数据经后处理,获得肾脏血流灌注图像和时间?密度曲线(TDC),通过感兴趣区获得皮髓质的不同灌注参数,包括肾血流量(BF)、肾血容量(BV)、肾脏对比剂平均通过时间(MTT)和肾脏强化高峰时间(TTP)。结果60岁以上组肾皮质的BF值较40~59岁组明显减低,差异有统计学意义(P<0.05);MTT值在60岁以上组有增加趋势,但与40~59岁差异均无统计学意义(P>0.05)。肾髓质灌注参数两组间差异均无统计学意义(P>0.05),但BV和MTT值在60岁以上组有增加趋势。结论年龄老化对肾脏血流的影响主要是皮质BF值显著下降;为临床了解肾脏病理生理变化提供量化依据。Objective To evaluate the difference of renal blood flow perfusion parameters with multislice spiral computed tomographic in the healthy over 60 years older. Methods 30 healthy people were divided into two groups by age : Ⅰ,40 - 59 years, Ⅱ,60 - 79 years, eachwere 15 cases. The renal perfusion scan was cartied out with 16 row detector spiral computed tomographic. The data were analyzed by CT perfusion software,obtained the perfusion picture and the time-density data, then get the perfusion parameters by regions of interest. Results The BF value of renal cortex in over 60-years-old group was smaller than the 40 to 59-years-old group, the difference was statistically significant( P 〈 0. 05 ) ; changes in the MTT value had enhancing, but there was no significant difference( P 〉 0. 05 )in two groups. The perfusion parameters of medulla were compared between the two groups, there was no significant difference (P 〉 0. 05 ) , but BV and MTT value had increasing. Conclusion CT perfusion imaging showed the effects of aging on renal blood perfusion lie on renal cortex, and the BF value appeared obvious decreased. CT perfusion imaging provided the quantification basis for the clinical understanding the pathology or physiology change of kidney.
