摘 要:目的调查武汉市和长沙市医务人员手的卫生状况并评价所用含抗菌和消毒成分洗手液的消毒效果。方法用问卷法调查医护人员的手卫生状况;用悬液定量杀菌试验、手现场消毒试验测试洗手液杀灭微生物的效果。结果武汉市和长沙市被调查的医务人员中,在接诊每例患者前均洗手的比例分别为57.9%和75.7%;医院使用了洗手液的比例为75.0%和68.0%;在悬液定量杀菌试验中,含三氯生、氯己定和聚己缩胍盐酸聚六亚甲基双胍的手消毒液对白色假丝酵母菌菌悬液消毒作用1min,杀灭对数值均>4,在现场消毒试验中,用2g/L含盐酸聚六亚甲基双胍洗手液和1.3g/L含苯扎氯铵的洗手液洗手,受试人员的细菌杀灭对数值均>1。结论需加强武汉市和长沙市医务人员卫生洗手重要性的认识和手卫生消毒措施的建立,三氯生类、苯扎氯铵类、盐酸聚六亚甲基双胍类手消毒液对自然菌和真菌均有良好的杀灭效果。OBJECTIVE To investigate hand hygienic status of the medical staff in cities of Wuhan and Changsha and evaluate disinfection effects of hand-washing solutions with antibacterial ingredients in the hospitals. METHODS A detailed questionnaire was used to interview the participants to collect information on personal hand hygiene. The suspension quantitative germicidal test and on-the-spot trial were used to evaluate antibacterial effects of used hand-washing solution in hospitals. RESULTS Among the medical staff studied, there were 57.9% in Wuhan and 75. 7% in Changsha washed their hands after they treated every patient, respectively. Among hospitals studied, 75.0% of medical staff in Wuhan and 68.0% of medical staff in Changsha used hand-washing solutions. The results showed that log values of killing Candida albicans were 〉4 for all hand-washing solutions containing either triclosan or chlorhexidine and polihexamide [poly (hexamethylene biguanide)hydrochloride] intergradients after they were added into the suspended solution of C. albicans. Log values of killing C. albicans all were ~ 1 when hand-washing with either 2g/L and 1. 3g/L benzalkonium chloride. CONCLUSIONS Better disinfection effects against bacteria and fungi are observed for hand-washing solution with either benzalkonium chloride, triclosan, chlorhexidine or polyhexamide. It is necessary to emphasize the importance of personal hand hygiene for these medical staff. Relevant sanitation measures on hands disinfection of medical staff should be reinforced in the hospitals.
