输尿管镜下钬激光治疗输尿管结石并发狭窄64例体会

摘　　要：目的:探讨输尿管镜下钬激光治疗输尿管结石并发狭窄的有效性及安全性。方法:对64例输尿管结石并发狭窄病人采用输尿管镜下钬激光内切开和碎石术,术中留置F8双"J"管或双F5双"J"管,术后2～3个月拔出。结果:64例病人中60例一次手术成功,2例改开放手术,2例因结石上移而仅行内切开术,术后随访2～10月,58例治愈和好转,有效率为90.6%。结论:输尿管镜下钬激光能同时治疗输尿管结石和狭窄,创伤小、疗效好、并发症少。Objective： To investigate the efficacy and safety of holmium laser on endoscopic treatment of ureteral calculi combining with stricture. Methods ： 64 patients with ureteral calculi combining with stricture received ureteral endoincision and lithotripsy by using holmium laser under ureteroscope,F8 double J tube or two F5 double J tube was placed in the ureters after the operation and left indwelling for 2 to 3 months. Results.. The operation succeeded in 60 among 64 cases, 2 cases received open surgery,and other 9. cases only received ureteral endoincision because the stones moved to pelvis. Follow up was available for 2 to 10 months, 58 （ 90. 6% ） were cured and improved after the treatment. Conclusion：Under ureteroscope holmium laser therapy can treat calculi combining with stricture of ureter at same operation,and is effective, less invasive with fewer complications.
