摘 要:近年来研究发现,贝特类调脂药对肾脏病理生理有广泛的作用.通过调脂作用贝特类药可抑制脂质诱发的肾脏足突细胞的砬激反应和凋亡。另外,贝特类药可促进肾脏一氧化氮等合成,改善血管内皮功能;抑制核因子-κB炎性反应通路及下调NAD(P)H氧化酶、血浆纤溶酶原激活物抑制剂-1、组织因子基因表达,起到抗炎、抗氧化、抗血栓作用:通过上述机制贝特类药发挥改善蛋白尿、防止肾损伤的作用。虽然贝特类药对肾小球滤过率无明显不良影响,但有升高血肌酐的报道,其与肾功能的关系、机制和远期预后尚未明确,需进一步研究.Recent studies show that fibrate plays a wide role in the pathophysiology of kidney. Fibrate inhibits stress response and apoptosis of podncytes via the downregulation of lipid. On the other hand, fibrate plays an important role in anti-oxidant stress,anti-inflammation and anti-thrombus in non-lipid regulation way,including inducing the geueration of NO to improve endothelial function, inhibiting the pathway of NF-κB,downregulating gene expression of NAD(P) H oxidase, PAI-1, tissue factor. So fibrate protects renal function. Although fibrate has no negative effect on glomerular filtration rate,it was reported to increase serum ereatinine level sometimes. The relation between fibrate and renal function,its mechanism and the long-term effect remain unknown and need further investigation.
