摘　　要：目的回顾性分析37例主动脉窦瘤破裂的外科治疗。方法主动脉窦瘤修补15例直接缝合,22例用涤纶片修补内破口;行主动脉瓣置换术9例,主动脉瓣成形术3例;VSD直接缝合4例,补片修补15例。结果全组死亡1例。35例随访6个月-5年,无残余分流等并发症,心功能恢复良好。结论主动脉窦瘤破裂一经确诊,应尽早手术,预后良好。对于严重主动脉瓣关闭不全,应同期行主动脉瓣置换术或主动脉瓣成形术。Objective To summarize the experience of the surgical treatment of ruptured aneurysm of aortic sinus.Methods From May 2001 to August 2008,31 cases with aneurysm of aortic sinus originated from right coronary sinus,6 originated from nocoronary sinus;19 cases combined ventricular septal defect,20 combined aortic insufficiency.15 cases received direct sutures of the aneurysm,22 were repaired with a patch;9underwent aortic valve replacement and 3 underwent aortic valvuloplasty;VSD was direct sutures in 4 cases,repaired with a patch in 15 cases. Results One died in perioperative period. 35 cases were followed up for 6 months - 5 years, cardiac function of the survivors recovered well. No residual shunt occurred. Conclusion Once ruptured aneurysm of aortic sinus confirmed, should be operated as soon as possible, and cases with severe aortic insufficiency should be eventually require aortic valve replacement or aortic valvuloplasty.
