银屑病患者外周血CD14^+单核细胞TLR4的表达及其与抗链球菌溶血素O的相关性

摘　　要：目的:检测寻常型银屑病(PV)患者外周血CD14^+单核细胞Toll样受体4(Toll-like receptor,TLR4)的表达,分析其与血清抗链球菌溶血素"O"(ASO)阳性率的相关性。方法:采用流式细胞术检测30例PV外周血CD14^+单核细胞TLR4的表达,采用乳胶凝集法测定血清中ASO滴度。以30例正常人作为对照。结果:PV患者外周血中CD14^+单核细胞TLR4的表达及ASO阳性率较正常对照组明显增高(P<0.01),PV患者ASO阳性者TLR4表达水平明显高于ASO阴性者(P<0.05)。进行期与静止期的PV患者外周血中CD14^+单核细胞TLR4的表达及ASO阳性率差异有显著性(P<0.05)。结论:TLR4及其介导的天然免疫应答在PV发病中可能起重要作用,其机制可能与链球菌等诱发机体免疫应答有关。Objective： To explore the expression of Toll - like receptor 4 （TLR4） in human peripheral blood CD14^＋ mononuclear cell and its correlation with antistreptolysin （ASO） in patients with psoriasis vu]garis （PV）. Methods： The level of TLR4 in human leucocytes was measured by flow cytometry in 30 PV patients and 30 normal controls. The level of serum ASO was measured by latex agglutination assay. Results： The expression of TLR4 in human leucocytes and the sera level of ASO were significantly increased in PV compared with normal controls （ P 〈 0.01）, and there was significant difference between the active stage of PV and inactive stage （P 〈 0.05）. The expression of TLR4 in psoriatic patients with positive ASO was higher than that in patients with negative ASO. Conclusions： TLR4 and the TLR4 - mediated innate immune response may play an important role in the pathogenesis of PV, which may be through recognizing the invading of streptococci and inducing the excretion of inflammatory factors.
