摘 要:目的探讨超声检查在汶川地震伤员救治中分诊的价值。方法回顾性地分析了超声检查在汶川地震伤员分诊中的应用方法和具体作用。结果3307例地震伤员中36.50%(1207/3307)接受了超声检查。所用超声检查方法主要为“创伤重点超声评估法”(FAST)。超声筛查出腹腔积血16例、胸腔积液(血)45例、肢体软组织深部血肿12例。超声检查阴性的患者中有5例证实有内脏损伤。超声直接相关病变超声阳性检出率为6.05%(73/1207),没有发现假阳性,假阴性率为0.45%(5/1123)。结论超声检查是震后最主要的创伤筛查方法,能够快速准确地分诊伤员,在大批地震伤员救治中发挥了重要作用。Objective To investigate the role of uhrasonography in triage in the rescue of earthquake victims. Methods Uhrasonography by the method of focused assessment of sonography for trauma (FAST) was used in triage among the Wenchuan earthquake victims. The resultant data were analyzed retrospectively. Results In triage 1207 of the 3307 wounded patients (36.5%) received ultrasound examination by the FAST method. Hemoperitoneum was found in 16 cases by ultrasound screening. In addition, 45 cases of hemothorax and 12 cases of deep hematoma of soft tissue were detected. Visceral injury was confirmed by surgery or CT later in 5 cases negative to ultrasonography. Trauma-associated pathology was detected in 73 patients (6.05%) by ultrasound screening. The false negative rate of uhrasonography was 0.45% (5/1 123) and no false positive cases was found. Conclusion A primary rapid examination means of trauma after earthquake, uhrasonography can quickly and accurately sort out the patients and play an important role in the rescue of a large number of earthquake victims.
