斯奇康治疗儿童支气管哮喘合并反复呼吸道感染疗效观察

摘　　要：目的探讨支气管哮喘合并反复呼吸道感染（RRI）经规律吸入糖皮质激素（ICS）控制不理想的儿童联用斯奇康（卡介菌多糖核酸）的临床疗效。方法将2007年9月至2009年3月笔者所在医院儿科门诊收治的60例哮喘合并RRI经系统ICS治疗控制不理想的患儿，根据入院先后分为治疗组（n=30）与观察组（n=30），观察组给予抗炎及一般对症治疗，治疗组在一般对症治疗的基础上加用斯奇康治疗，观察两组患儿RRI及哮喘临床症状、血清IgA、IgG、IgM及CD3＋、CD4＋、CD8＋等免疫功能的变化。结果与对照组比较，斯奇康治疗组呼吸道感染次数、发热天数、抗生素使用天数、咳嗽及气喘发作天数均减少，差异有显著性（P均〈0．05）。斯奇康治疗组治疗后血清IgA、IgG、及CD3＋、CD4＋、CD8＋百分率均较治疗前显著升高（P〈0．05），而对照组治疗后免疫功能指标无明显变化（P〉0．05）。结论哮喘合并RRI经ICS控制不理想儿童联用斯奇康治疗能有效预防RRI，从而减少哮喘发作的次数，减轻哮喘严重程度。Objective To Observe the effects of polysaccharide nucleic acid fraction of bacillus guerin（ BCG - PSN） on asthmawith recurrent respiratory infection（RRI） in children. Methods 60 patients with asthma and RRI were divided into treatment group（ n = 30）and comparison group（ n = 30）separately. The comparison group accept normal treatment. The treatment group was treated with normal treatment combined with BCG -PSN intramuscular injection. The clinic symptoms ,serum IgA ,IgG,IgM levels and percentage of CD3 ＋ ,CD4 ＋ ,CD8 ＋ were observed. Results Frequency of respiratory infection, days of fever and antibiotics consumption, day of cough and wheeze in treatment group were significantly less than comparison group（ P 〈 0.05 ）. In the treatment group , after the treatment of the patients for six months, serum IgA, IgG levels and percentage of CD3 ＋ , CD4 ＋ , CD8 ＋ were significantly raised than before the treatment （ P 〈 0.05 ）, while they were unchanged in the comparison group（ P 〉 0.05）. Conclusion The present data shows that BOG - PSN has significant clinical effectiveness on asthma with recurrent respiratory infections in children is associated with decrease the frequency of respiaratory infection, occurrence of asthma outbreak and severity of asthma.
