摘 要:目的观察比较盐酸戊乙奎醚和阿托品用于新生儿手术全身麻醉前用药的临床效果。方法30例新生儿手术患者,根据麻醉前用药的不同随机分为盐酸戊乙奎醚组(P组)和阿托品组(A组),每组15例,分别在麻醉开始前30min肌肉注射盐酸戊乙奎醚或阿托品0.02mg/kg。记录给药前(T0)、给药后15min(T1)和30min(T2)时患儿的血压(BP)、心率(HR)、脉搏血氧饱和度(SPO2)及T0、T1、T2和气管导管拔除时(T3)患儿面红、腺体分泌情况。结果2组患儿性别、年龄、体重和手术时间无统计学差异(P>0.05);P组术前给药后HR、BP无明显改变(P>0.05),A组给药后HR均明显增快,与P组比较差异有统计学意义(P<0.05);P组与A组比较,气管拔管时分泌物量明显减少(P<0.05),且面红发生率较低(P<0.05)。结论盐酸戊乙奎醚具有显著的心率稳定作用及强大持久的腺体分泌抑制作用,作为术前用药效果满意,优于阿托品,可安全用于新生儿手术麻醉前给药。Objective To investigate and compare the clinical effect of Penehyclidine Hydrochloride used as premedication of general anesthesia to Atropine on neonates's surgery. Methods In a randomized study, 30 neonates patients were divided into two groups: group P (Penehyclidine Hydrochloride group, n=15) and group A (Atropine group, n=15). Group P was injected muscularly with 0.02mg/kg Penehyclidine Hydrochloride 30 minutes before general anesthesia, while group A were administered with 0.02mg/kg Atropine respectively. The heart rate(HR), blood pressure(BP) and pulse oxygen saturation(SPO2) were recorded before medication(T0) and 15min(T1), 30 min(T2) after medication. The secretion of respiratory tract, facial flushing at T0, T1, T2 and 33( tracheal catheter extracting) were evaluated. Results There was no significant difference between the sex, age, weight and surgery time of the neonates patients in two groups(P〉0.05); and no obvious change appeared in the heart rate of the patients in group P(P〉0.05). While the heart rate of the patients in the group A significantly increase comparing to the basal value 15 and 30 minutes after medication (P〈0.05). Moreover, secretion of respiratory tract was prominently decreased in group P at 33 comparing to group A (P〈0.05). Conclusion Penehyclidine Hydrochloride has significant effects of inhibiting secretion of salivary glands, maintaining cardiovascular stability, and is a proper premedication of surgery in neonates patients undergoing general anesthesia.
