下睑眶隔脂肪异位移植及眶肌筋膜韧带悬吊术的临床应用

摘　　要：目的解决下眶缘凹陷及颧部外侧组织松弛下垂至鼻唇沟过深，使睑袋矫正术后的效果更加完善。方法将眶隔脂肪及包裹其表面的筋膜间断缝合固定于下眶缘骨膜上，以填充下眶缘之凹陷。游离长1cm的眶肌筋膜韧带（勿损伤此韧带），将其缝合固定于眶外缘骨膜处。结果本组128例患者，术后随访6个月，126例效果满意，2例因自身眶内脂肪量不足，致下眶缘凹陷填充欠佳，经过补添胶原蛋白后效果满意。结论自体下睑眶隔脂肪异位移植，可避免从其他部位抽取自体脂肪移植于受区而引起脂肪被吸收的现象，同时也避免增加新的创伤。该手术创伤小，时间短，术后恢复快，效果理想，值得在临床中推广应用。Objective To perfect the cosmetic results of lower eyelid blepharoplasty. Methods Hollow of the inferior orbital margin was filled by sewing interruptedly orbital fat and the fascia covering the orbital fat on periosteum of inferior orbital margin. Orbital muscle fascia ligament was separated for 1 cm length and then was sutured and fixed on the on periosteum of orbital exterior margin. Results After 6 months follow-up, all 126 cases were satisfied with the results except 2 cases showed hollow filling of inferior orbital margin below the mark because of the lack of inner orbital fat, which was recovered by filling albumen. Conclusion Heterotopic transplantation of autoallergic inferior eyelids fascia palpebralis fat can avoid being absorbed of fat and more cuts and wounds. The method worth clinical spreading because of slight wound, shorter operation time, the fast recovery phase and perfect cosmetic results.
