摘  要:目的了解本社区新生儿听力障碍的发生率和影响新生儿听力损害的高危因素。方法2008年1月至2008年12月在我院出生的正常新生儿1224例,其中1103例在出生后3d以耳声发射进行初筛,42d复筛,未通过者3个月再筛,仍未通过者经听性脑干反应(auditory brainstem response,ABR)检查诊断是否有听力障碍。结果除拒绝筛查与其他原因未筛查者121例,实际筛查1103例,筛查率90.11%(1103/1224),通过962例,初筛通过率87.22%(962/1103),141例需复筛者,实际筛查123例,通过108例,复筛通过率87.8%(108/123),15例需再筛查者,实际筛查10例,通过7例。ABR检查18例,最后确诊听力障碍2例,发病率为1.81‰(2/1103)。结论本社区新生儿听力障碍的发生率为1.81‰,听力障碍高危因素以孕早期病毒感染、新生儿窒息、胎儿宫内窘迫、高胆红素血症、孕妇中重度贫血、妊高症重度子癫为主。Objective To investigate the incidence of neonatal hearing impairment in our district. Methods One thousand two hundred and twenty-four neonates were born from January to December in 2008 ,in which 1 103 cases were tested with otoacoustic emission (OAE) for 3 times (3 days, 42 days and 3 months after born). Those who didn't refer the OAE test were tested with auditory brainstem response(ABR) for the final diagnosis. Results Only 962 newborns(87.22% ,962/1 103) referred the initial screening of a total of 1 103 cases. For the 141 neonates who needed the second screening,only 123 cases were tested, in which 108 cases referred. For the 15 cases who needed the third screening,only 10 cases were tested, in which 7 cases referred. For the 18 cases tested by ABR, 2 cases (1.81%o) were diagnosed as hearing impairment. Conclusions The incidence of neonatal hearing impairment in the district was 1.81%0. The high risk factors might conclude viral infection at early gestation, neonatal asphyxia, fetal distress in uterus, hyper- bilirubinemia, gravida moderate or severe anemia and pregnancy-induced hypertension syndrome.
