摘  要:目的探讨蛞蝓胶囊对实验性肺气肿合并肺动脉高压动物的治疗作用。方法用弹性蛋白酶结合烟熏复制金黄地鼠肺气肿并肺动脉高压模型,将造模成功的金黄地鼠随机分为模型对照组,氨茶碱组,蛞蝓低、中、高剂量组,每组15只,分别给予等容量生理盐水,氨茶碱120mg·kg-1.d-1,蛞蝓胶囊23、70、210mg·kg-1.d-1灌胃给药,另取正常金黄地鼠15只作为正常对照组予等容量生理盐水灌胃。给药30d后,测定右心室收缩压(RVSP)、右心室肥厚指数(RVHI)、肺泡灌洗液中巨噬细胞数,并观察肺脏组织病理学特征。结果与正常对照组比较,模型对照组RVSP、RVHI和灌洗液中巨噬细胞数均增高,差异显著(P<0.05)。治疗30d后,与模型对照组比较,蛞蝓高、中剂量组肺气肿病变明显减轻,肺内细小动脉病变明显改善;蛞蝓高、中剂量组RVSP、RVHI和灌洗液中巨噬细胞数水平均低于模型对照组,有显著差异(P<0.05)。结论蛞蝓胶囊高、中剂量对实验性肺气肿合并肺动脉高压具有较好治疗作用,抑制肺泡巨噬细胞渗出可能为其作用机制之一。AIM To investigate the therapeutic effect of slug capsule on experimental emphysema complicated with pulmonary artery hypertension (PAH). METHODS Intratracheal infusion with elastase combined with passive smoke inhalation was adopted to make the models of hamster with emphysema complicated with PAH. The models were allocated according to different treatment into model group (equivalent normal saline gastric infusion), aminophylline group (aminophylline 120 mg·kg^-1·d^-1), slug low dosage group (slug 23 mg·kg^-1·d^-1), slug middle dosage group (slug 70mg·kg^-1·d^-1) and slug high dosage group (slug 210 mg·kg^-1·d^-1) randomly, 15 in each. Another 15 normal hamsters were into normal control group with equivalent normal saline gastric infusion. By the end of 30 days of treatment, right ventricular systolic pressure (RVSP) was measured, and then all the animals were sacrificed for measuring the right ventricle hypertrophy index (RVHI), as well as the number of macrophage in alveolar lavage fluid; and pulmonary tissues was observed under light microscopy with HE staining. RESULTS Compared with the normal control group, RVSP, RVHI and macrophage cells in alveolar lavage fluid of the'model group increased significantly (P 〈 0.05). In comparison with the model group, the pathological changes of emphysema and pulmonary small artery of the slug high dosage group and the middle dosage group were markedly improved. RVSP, RVHI and macrophage in alveolar lavage fluid of the slug high dosage group and the middle dosage group decreased significantly in comparison with that of the model group (P 〈 0.05). CONCLUSION Slug capsule is an effective concentration dependent remedy for experimental emphysema complicated with PAH through probable mechanism of inhibiting exudation of pulmonary alveolar macrophage.
