AKA模拟抗原对RF阴性类风湿关节炎的诊断价值

摘　　要：探讨利用噬菌体随机肽库筛选到的AKA相关模拟抗原对RF阴性类风湿关节炎患者的诊断价值。将间接荧光法检测AKA阳性的RA患者混合血清,经33.3%饱和硫酸铵盐析纯化IgG,以此多克隆抗体为配基,对噬菌体递呈的随机12肽库进行淘筛,将筛选到的阳性噬菌体克隆调整至一定浓度包被酶标反应板,通过间接ELISA法检测RA血清标本,与RF检测结果进行比较。结果是筛选到的AKA模拟抗原对RA诊断的特异性为97.4%,敏感性为56.7%;RF阴性患者中AKA阳性率为52.4%。因此,筛选到的AKA模拟抗原对RF阴性的RA患者是一个有价值的补充诊断指标。The purpose of this study is to obtain the mimotope recognized by AKA by screening a phage random peptide library, and to evaluate its diagnostic value, IgG was purified from AKA-positive pooled serum of the RA patients by 33. 3% （NH4）2 SO4 and AKA was measured with indirect immunofluorescence technique. Then the antibody was used as the ligand for biopanning of a 12-phage random peptide library. The serum of patients with RA, SLE and normal people were detected with the positive clone by indirect ELISA. After 4 rounds of screening, the peptide of mimotope associated with RA was. Its diag nostic specificity, sensitivity were 97. 4% and 56. 7% respectively. In patients of RA with negative RF, the sensitivity of AKA was 52.4%. The mimotope amino acid sequence of minotope was found to be QSESAGPTTSRR, similar to 137-148 amino acid sequence of human filaggrin.
