摘  要:目的了解医院重症监护病房(内科ICU)感染患者铜绿假单胞菌耐药谱的变化,指导内科ICU合理用药。方法收集2007年1月-2008年12月内科ICU送检的感染患者标本,分离铜绿假单胞菌,分析各类标本分离的铜绿假单胞菌构成比;采用MicroScan NC31、NC33鉴定药敏复合板进行鉴定和药敏试验,按美国临床实验室标准化委员会(CLSI)标准对常用的8种抗生素进行耐药性分析。结果共分离出131株铜绿假单胞菌,主要来源于痰液,占80.2%。铜绿假单胞菌对头孢他啶的敏感率为77.2%,对环丙沙星的敏感率为51.4%,且呈上升趋势;对庆大霉素、亚胺培南、氨曲南和哌拉西林的敏感率在43.8%~51.8%之间,对复方磺胺的敏感率为2.2%,且呈下降趋势。有六种药物耐药率呈上升趋势。结论铜绿假单胞菌耐药现象严重,ICU应注意铜绿假单胞菌耐药性的监测。Objective To analyze the resistance spectrum of Pseudomonas aeruginosa in infectious patients in an intensive care unit (ICU) and to instruct physicians in the rational administration of drugs. Methods P. aeruginosa strains were collected from ICU specimens over the last two years and the constituent ratio and the resistance status of the P. aeruginosa strains were analyzed. MicroScan NC31 and NC33 composite boards were used to identify strains and perform a drug sensitivity test. To analyze the resistance spectrum of P. aeruginosa with respect to 8 common antibacterials according to CLSI. Results Sensitivity of P. aeruginosa to CAZ and CIP was 77.2% and 51.4%, respectively, and tended to rise over the last two years. Sensitivity of P. aeruginosa to GEN, IMP, ATM, and PIP was 43.8% -51.8% and tended to decrease. Sensitivity of SXT was the lowest, only 2.2 %, and tended to decrease. The rate of resistance for six antibiotics tended to increase over the last two years. Conclusions The resistance of P. aeruginosa is a serious issue, and ICUs should monitor its drug resistance status.
