摘 要:目的探讨鼻饲胃管外测量长度(耳垂-鼻尖-剑突)与食管心房调搏电极插至最佳位置所需长度之间的关系,以指导临床医护人员在食管心房调搏术中快速定位电极。方法将80例行食管心房调搏的患者,当食管心电图振幅最大P波为正负双向波时,记录食管电极插入长度,并测量耳垂-鼻尖-剑突的长度及鼻尖-耳垂的长度,鼻饲胃管的长度减去食管电极最佳位置长度之差,与耳垂-鼻尖长度相比较。结果鼻饲胃管外测长度为(50.18±2.68)cm,食管电极插至最佳位置所需长度为(39.80±2.58)cm,两种测量长度的差值(10.22±1.61)与耳垂-鼻尖(9.98±1.56)长度比较,差异无统计学意义(P>0.05)。结论食管电极插至最佳位置的深度可以用鼻尖到剑突的长度来计算。Objective To explore the relationship between the length from earlobe via nose tip to sternum(ENS), which was commonly used as optimal depth of insertion (DOI)of a nasal gastric tube and the DOI of electrodes during transesophageal atrial pacing, and to provide information for quick location of electrodes during the procedure. Methods Totally, 80 patients were receiving the procedure of transesophageal atrial pacing. When the esophageal electrocardiogram showed that a maximum amplitude P wave turned into a bidirectional wave, the DOI of electrode was recorded, and meanwhile, the length of ENS, and the length from nose tip to earlobe (NE) were noted, too. The result of the length of ENS minus the DOI of electrode was compared with the length of NE. Results The length of ENS reached (50.18±2.68)cm, and the DOI of electrode(39.80±2.58)cm. The result of the length of ENS minus the DOI of electrode had no significant difference with the length of NE(P〉0.05). Conclusion The optimal DOI of electrode during transesophageal atrial pacing equals to the length from nose tip to sternum, which can act as a short cut in the location of electrode.
