经胸断流大网膜包肺术治疗门脉高压症14例疗效分析

摘　　要：1989年4月～1992年12月对14例门脉高压症合并食管胃底静脉破裂出血者施行经胸断流、大网膜包肺术。手术死亡率为21．4％，其中肝功能Ⅰ、Ⅱ级为12．5％，Ⅲ级为33．3％，急症手术死亡率33．3％。5年生存率643％，随访无再出血和肝性脑病发生，B超测量门静脉直径逐渐缩小并维持在正常水平。Form April 1989 to December l992, 14 patients who sufferred from upper GI bleedingdue to portal hypertention were treated with the method of transthoracic disconnective operation withgreater omentum wrapping the lung. The operation mortality was 2l. 4 %, among which Child's A andB patient's was l2. 5% (l/8), Child's C patients' 33. 3 % (2/6). The bleeding control rate was 100%.No recurrent bleeding or hepatic encephalopathy occurred in the follow - up- Ultrasonic examinationshowed that the diameters of the portal veins reduced gradually and then maintained at normal levels. Thismethod was a kind of ' combined operation' with devascularization thorough, shunt reliable, and its usedeserved spreading.
