摘 要:目的研究MSCT双期增强扫描对腮腺良恶性病变的诊断及鉴别诊断价值,并总结各病种的双期增强CT表现特征。方法对病理证实的13例良性混合瘤、10例腺淋巴瘤、2例嗜酸性腺瘤、2例单纯性腺瘤、17例恶性肿瘤和6例慢性炎症共50例腮腺良恶性疾病病例进行腮腺MSCT平扫和注药后30秒、120秒双期增强扫描,测量各期的病灶CT值并进行统计学分析,总结不同性质腮腺病变的双期增强CT特征并评价其诊断价值。结果恶性肿瘤与良性混合瘤的双期增强值之间差异无统计学意义(P>0.05),而二者与腺淋巴瘤的双期增强值均存在差异且具有统计学意义(P<0.05)。肿瘤包埋和/或破坏邻近血管、肿瘤内低密度区呈中央分布、肿瘤同时累及腮腺深浅叶三种CT征象对腮腺恶性肿瘤诊断的敏感性、特异性分别为88.24%、90.00%、76.47%和100%、87.50%、92.59%。延迟强化征象、延迟排空征象分别对良性混合瘤、腺淋巴瘤的诊断敏感性和特异性为90%、92.31%和88.24%、85.71%。结论MSCT双期增强扫描对腮腺良恶性病变的诊断及鉴别诊断具有重要价值。Objective To study the diagnosis and differential diagnosis value of dualphase enhanced multi-slice spiral CT (MSCT) in parotid gland benign and malignant lesions, and its characteristics. Methods 50 benign and malignant Lesions those were confirmed by pathology were undergone unenhanced CT and enhanced CT after the administration of contrast material was performed at 30 seconds and 120 seconds. Lesions included 13 benign pleomophic adenomas, 10 Warthin's tumors, 2 oncocytoms, 2 simple adenomas, 17 malignant tumors and 6 chronic inflammations. CT values were calculated and analysed statistically. Dual-phase MSCT characteristics on parotid gland benign and malignant lesions were evaluated. Results CT value on enhanced MSCT were of no statistical difference between malignant tumors and benign pleomophic adenomas(P〉0.05)and both of them were different from Warthin's tumors statistically (P 〈0.05). The Sensitivity and the specificity of diagnosis in parotid gland benign and malignant tumors were 88.24%, 90.00%, 76.47% and 100%, 87.50%, 92.59% by characteristics of vessels adjacent tumors invased and(or) destructed, hypo-attenuation district locating in tumor center, both deep and superficial lobe of parotid gland invased. The Sensitivity and the specificity of diagnosis in parotid gland benign pleomophic adenomas and Warthin's tumors were 90%, 92.31% and 88.24%, 85.71% by delayed enhancement or washout. Conclusion Dual-phase MSCT scan was helpful for the diagnosis and differential diagnosis of parotid gland benign and malignant lesions.
