中晚孕期孕妇血清抑制素A水平及其与胎儿非整倍体的相关性初探

摘　　要：目的:建立正常孕妇血清抑制素A在孕中晚期各孕周的参考值,探讨孕妇血清抑制素A水平异常与胎儿染色体非整倍体可能存在的相关关系。方法:采用酶链免疫吸附技术(ELISA)试剂盒测定孕妇血清抑制素A浓度,按完全孕周分别统计出各孕周正常孕妇的血清抑制素A中位数值;比较唐氏综合征和18-三体综合征妊娠的血清抑制素A水平异常。结果:昆明地区孕14～36周共计2592例正常单胎妊娠孕妇的血清抑制素A浓度中位数分别是:351.0、315.5、259.0、265.0、281.0、265.5、302.0、326.0、371.0、423.5、417.0、450.0、532.0、685.5、703.0、628.0、838.0、926.0、743.0、882.0、1340.0、1260.0、1490.0pg/ml。经产前诊断证实的9例胎儿染色体数目异常中有6例孕妇血清抑制素A水平异常。结论:孕妇血清抑制素A水平异常与胎儿染色体非整倍体存在一定的相关性,在血清标记物中加入血清抑制素A有可能提高产前筛查的检出率;建立各孕周孕妇血清抑制素A的正常值,可以为本地区产前筛查以及其他妊娠相关疾病的研究提供参考。Objective： To establish the serum inhibin -A referential values of normal pregnant women at each second and third - trimester week, explore the correlation between serum inhibin A level of pregnant women and fetal aneuploid. Methods： Serum inhibin A levels of the women were detected by ELISA, and the median values for each gestational week were summarized, respectively. The maternal serum inhibin A levels of Down syndrome and trisomy 18 syndrome were compared. Results： 2 592 serum samples from normal, singleton pregnant women at 14 -36 gestational weeks were tested. The median values for each completed gestational weeks were 351.0, 315.5, 259.0, 265.0, 281.0, 265.5, 302.0, 326.0, 371.0, 423.5, 417.0, 450.0, 532.0, 685.5, 703.0, 628.0, 838.0, 926.0, 743.0, 882. 0, 1 340. 0, 1 260. 0 and 1 490. 0 pg/ml, respectively. Abnormal maternal serum inhibin A levels were found in 6 cases among 9 cases with Down syndrome or trisomy 18 syndrome pregnancies verified by prenatal diagnosis. Conclusion： Abnormal maternal serum inhibin A levels have some associations with the fetal chromosomal aneuploid. It is possible to increase the detection rate when inhibin A is added into the serum markers in the prenatal screening. Setting up the normal values of maternal serum inhibin A at various gestational weeks is valuable for the researches on prenatal screening and gestation - related diseases in local populations.
