摘　　要：【目的】探讨无创通气（NPPV）对高龄慢性心力衰竭（CHF）患者血浆脑钠肽（BNP）水平的影响。【方法】连续入选2007年6月至2008年6月在本院住院的高龄CHF患者60例，随机分为NPPV组（A组）和基础治疗组（B组），入选患者均行超声心动图检查和血浆BNP的测定。【结果】高龄CHF患者治疗前的血浆BNP水平明显升高，A组以及B组血浆BNP较治疗前显著下降（P〈0．05），且治疗后，A组较B组下降程度更大。A组治疗后心功能Ⅳ级者下降较Ⅲ级下降更为显著（P〈0．05）。【结论】在基础治疗基础上辅以NPPV治疗高龄CHF患者，改善心功能作用更显著，对于心功能Ⅳ级CHF患者较Ⅲ级效果更为显著。[Objective]To investigate the effect of non-invasive positive pressure ventilation（NPPV） on the change of plasma B-type natriuretic peptide（BNP）in aged patients who suffered from congestive heart failure（CHF）. [Methods] Sixty aged patients with CHF were analyzed frorri June 2007 to June 2008. They were divided randomly into NPPV group and conventional treatment group. The parameters of echocardiography and plasma BNP were ana- lyzed. [Results]The levels of plasma BNP elevated significantly in aged patients with CHF. After treatment, BNP level in NPPV group was decreased more significantly than that in conventional treatment group. In NPPV group, BNP level was decreased more significantly in patients with NYHA heart function IV than that in patients with NYHA heart function Ⅲ. [Conclusion]NPPV treatment is a complement to conventional treatment. NPPV treatment combined with conventional treatment is more effective for improving heart function, and more effective in patients with NYHA heart function IV than in patients with NYHA heart function Ⅲ
