舒芬太尼、芬太尼用于断指再植术后镇痛的临床观察

摘　　要：目的:观察比较等效剂量舒芬太尼和芬太尼用于断指再植术后镇痛的效果。方法:将120例行断指再植术的患者随机分为两组,每组60例。术后镇痛A组患者采用芬太尼30ug.kg-1+昂丹斯琼8mg+生理盐水至100mL,B组患者采用舒芬太尼3ug.kg-1+昂丹斯琼8mg+生理盐水至100mL。观察术后8、16、24、48、72h的镇痛评分和镇静评分。结果:两组患者镇痛评分的比较差异有统计学意义(F=27.144,P=0.000);B组术后各时点VAS评分均低于A组(P=0.000)。两组整体镇静评分比较,差异有统计学意义(Z=30.75,P<0.0001);两组患者性别、年龄对镇静评分的影响比较,差异有统计学意义(P=0.0498,P=0.0009);两组患者体重对镇静评分的影响比较,差异无统计学意义(P=0.2881);两组术后各对应时点镇静评分比较,差异有统计学意义(P<0.05)。结论:断指再植术后,应用等效剂量的舒芬太尼较芬太尼镇痛镇静效果更满意,性别和年龄对镇静程度有一定影响,而体重对镇静程度无明显影响。Objective：To observe the analgesie effect of Sufentanil and Fentanyl on patients after replantation of amputated finger. Method： 120 patients were randomly divided into Fentanyl group（A） and Suifentanil group（B） ,60 patients in each group. Patients in Group A were administered with Veotanvl（30 ug·kg^-1） and Ondansetron（8 mg） in 100 mL Sodium Choride,while those in Group B were administered with Safentanil（ 3 ug·kg^-1 ） and Ondansetron（8 mg） in 100 mL Sodium Choride. The visual analog scale（VAS） and sedation score （SS） were recorded at 8,16,24,48,72 postoperative hours. Result：There were statistical difference in total VAS score between group A and B （F = 27. 144, P=0. 000）, the VAS score in group B was lower than that in group A at all the postoperative time -points （P = 0. 000）. There were statistical difference in total SS score between group A and B（ Z = 30.75, P 〈 0. 000 1 ）. There were statistical difference in the influence of sex and age on the SS score between the two groups（P =0. 049 8, P =0. 000 9） ,while there were no statistical difference in the influence of weight on the SS score between the two groups（P = 0.288 1 ）. There were statistical difference in the SS score between the two groups at each postoperative time-points （ P 〈 0. 05 ）. Conclusion： Sufentanil has better analgesic effect and sedative effect than Fentanyl on patients after replantation of amputated finger. Sex and age of patients have effect on sedative depth,while weight of patients have no effect on sedative depth.
