糖皮质激素联合生长激素治疗慢加亚急性肝衰竭临床观察

摘　　要：目的观察应用糖皮质激素联合生长激素治疗早、中期慢加亚急性肝衰竭患者的疗效。方法53例早、中期慢加亚急性肝衰竭患者被随机分为常规治疗组(28例)、糖皮质激素治疗组(14例)和糖皮质激素联合生长激素治疗组(11例)。结果53.6%(15/28)常规治疗组患者死亡,而21.4%(3/14)糖皮质激素治疗和18.2%(2/11)糖皮质激素联合生长激素治疗患者死亡(P<0.05);糖皮质激素联合生长激素治疗组患者感染和肝肾综合征的发生明显减少,血清白蛋白明显升高。结论在基础治疗的支持下,应用糖皮质激素治疗早、中期慢加亚急性肝衰竭患者可取得一定的效果,而加用生长激素后可明显减少严重并发症的发生,提高血清白蛋白水平。Objective To explore the effect of glucocorticoid combined with growth hormone in the treatment of patients with subacute-on-chronic hepatic failure. Methods 53 patients with subacute-on-cbronic hepatic failure were selected and 14 patients received treatment of glucocorticoid, l l received glucocorticoid combined with growth hormone and other 28 received conventional therapy. Result The survival rate,similar with that in glucucorticoid therapy （21.4%） in glucocorticoid combined with growth hormone treatment was 18.2%,much lower than in conventional group （53.6%,P〈 0.01 ）. Conclusion Glucocorticoid combined with growth hormone in treatment of patients with subacute-on-chronic hep- atic failure can effectively decrease the mortality rate.
