经皮激光椎间盘减压术与电磁场联合治疗腰椎间盘突出症的临床观察

摘　　要：目的探讨应用经皮激光椎间盘减压术(PLDD)与电磁场(EMFs)联合治疗腰椎间盘突出症(LDH)的临床疗效。方法将60例LDH患者随机分成治疗组和对照组,每组30例。治疗组行PLDD治疗,术后EMFs治疗;对照组行PLDD治疗,术后口服扶他林,连续2周。采用中华医学会脊柱学组腰背痛手术评定标准,于术后1周、1个月、3个月、6个月做疗效评定。结果术后1周治疗组疗效明显优于对照组,差异非常显著(P<0.01);术后1个月、3个月、6个月两组无显著差异(P>0.05),但治疗组优良率一直稳定高于对照组;治疗组反应性椎间盘炎发生率明显低于对照组,有非常显著差异(P<0.005)。结论本方法将微创技术与无创技术相结合治疗LDH能有效控制PLDD术后炎症反应,增强疗效,减少并发症,是治疗LDH的最佳方法之一。Objective To study the therapeutic effect of percutaneous laser disc decompression （PLDD） combined with electromagnetic fields （EMFs） in treating lumbar disc herniation （LDH）. Methods 60 cases of LDH were divided into 2 groups randomly： treating group and control group,30 cases in each group. The treating group were given the treatment of EMFs after received PLDD;the control group were given votalin orally after received PLDD for 2 weeks. The operation evaluation criteria of lumbago and backache of Chinese Medical Association Spine Group was taken to evaluate the therapeutic effect at 1 week, 1 month,3 months and 6 months postoperation. Results At 1 week postoperation,the therapeutic effect of the treating group was umch better than that of the control group,there were significant differences （P〈0.01）between the 2 groups;at 1 month,3 months and 6 months postoperation,there were no apparent differences （P〉0.05）between the 2 groups. But the excellent and good rate of the treating group was higher than that of the control group all the time. The occurrence rate of intervertebral discs reactive inflammation in the treating group was much lower than that of the control group,there were significant differences（P〈 0.005）. Conclusion PLDD combined with EMFs can control postoperative inflammation reaction in effect,improve the therapeutic effect,reduce the complications in treating LDH. It should be one of the best combined methods.
