下胫腓韧带的断层解剖学研究

摘　　要：目的:为临床下胫腓韧带损伤的影像诊断提供断层解剖学依据。方法:选取30例成年男尸右足踝标本,于自然松弛位(跖屈20°)经冰冻后制成断层标本,其中,冠状位10例、矢状位10例、横断位10例。观察下胫腓韧带出现的范围、附着及走行。结果:下胫腓前韧带在经前踝冠状面(宽5～8mm、高18～21mm)和经距骨顶横断面(长20～22mm、宽2～3mm)可清晰显示。下胫腓后韧带在经载距突后缘冠状面(宽8～10mm、高5～7mm)和经距骨顶横断面(长18～20mm、宽1～2mm)可显示。胫腓骨间韧带在经载距突后缘冠状面(上端宽4～6mm、下端宽1～2mm、高21～24mm)可显示。结论:选择合适的方位能清楚地显示下胫腓韧带,为下胫腓韧带损伤影像诊断提供断层解剖学依据。Objective： To provide a cross-sectional anatomic evidence for imaging diagnosis of infer-tibiofibular ligament injury in clinic. Methods： 30 samples of right ankle of dead cadavers of mature men, which were in natural loosening position （plantar flexed 20°） and were made for samples after freezing. Among the total samples, 10 cases were in coronal position, 10 cases were in anteroposterior axes position, 10 cases were in traverse position. The extent, insertio and courser of the infer-tabiafibular ligament were observed. Results： In the coronal position of antero-ankle （width 5-8 mm, height 18-21 mm） and traverse position of the top of the stragaloid bone （length 20-22 mm, width 2-3 mm）, the infer-tibiofibular ligament was well seen. In the coronal position of posterior edge of sustentaculum tali （width 8-10 mm, height 5-7 mm） and traverse position of the top of the stragaloid bone （length 18-20 mm, width 1-2 mm）, the infer-tibiofibular ligament was seen. In the coronal position of posterior edge of sustentaculum tali （width of superior extremity 4-6 mm, width of inferior extremity 1-2 mm,height 21-24 mm）,the ligment between tibiofibula was seen. Conclusion： The infer-tibiofibular ligament can be well shown in the right position, which provided a cross-sectional anatomic evidence for imaging diagnosis of infer-tibiofibular ligament injury.
