摘 要:目的:研究中药复方平肺口服液在预防大鼠放射性肺炎过程中对细胞因子的调节作用,为临床运用平肺口服液预防放射性肺炎提供实验依据。方法:SD大鼠30只随机分为正常对照组、模型组和平肺口服液组。X线全胸单次照射20Gy建立大鼠放射性肺炎模型,平肺口服液20g/(kg·d)于照射前一周开始灌服,每日一次,每周称体重调整一次给药剂量,共5周。分别于照射后4周和8周处死大鼠,摘取全肺,称湿重,计算肺指数;行肺脏组织学染色;Trizol法提取肺组织总RNA,RT-PCR法检测IL-6、IL-10和IFN-γ基因表达水平。结果:大鼠全胸照射后4周体重增长明显减少,肺指数升高,肺组织发生广泛炎症改变,IL-6和IL-10 mRNA表达升高,IFN-γ mRNA表达降低,与正常对照组相比有显著差异。平肺口服液灌服可抑制放射性肺炎大鼠体重增长缓慢的趋势(P<0.05),降低肺指数(P<0.05),减轻病理改变,并改善IL-6、IL-10和IFN-γ基因的异常表达(P<0.05,P<0.01,P<0.01)。结论:中药复方平肺口服液能预防大鼠放射性肺炎,其作用与调节Th1/Th2型细胞因子有关。Objective: To study the regulation of Pingfei Oral Liquid on cytokine expressions in rat radiation pneumonitis, one Traditional Chinese Medicine(TCM) recipe, in order to clinical useage of provide experimental foundations to this recipe's . Methods:30 rats were randomized into 3 groups, which were normal control group, model group and Pingfei group. A single dose of 20 Gy was delivered to the thoraces of Sprague-Dawley (SD) rats to set up the model of radiation pneumonitis. Pingfei Oral Liquid 20 g/kg/d was daily given orally from 1 week before irradiation until 4 weeks after irradiation,with dose adjustment according to rat weights once a week. Rats were killed at the 4th and 8th week respectively after irradiation. The whole lung was taken, the wet weight of lungs was measured and the lung index was calculated. Histological analyses with HE staining for the lower lobes of the right lung were carried out. Total RNA of the lung tissue was extracted using Trizol reagent for measurend of mRNA of IL-6, IL-10 and IFN-γ being confirmed by reverse transcription-polymerase chain reaction (RT-PCR). Results:4 weeks after whole thoracic irradiation, weight increases of the rats were diminished, and the lung indexes were raised. The lung tissue manifested wide inflammatory changes. The mRNA of IL-6 and IL-10 increased as well as that of IFN-γ decreased. All of the above changes were statistically different compared with those of the control. The rats fed with Pingfei Oral Liquid presented relatively better weight increases and lower lung indexes ( P 〈 0.05). Pathological manifestations and abnormal cytokine expression in the lung tissue were also ameliorated significantly. Conclusion: The rat radiation pneumonitis could be prevented by Pingfei oral liquid, one TCM recipe, which may be concerned with its immunoregulatory activity on Th1/Th2 type cytokines.
