腹腔镜手术治疗小儿肠套叠45例临床体会

摘　　要：目的:探讨腹腔镜手术在空气灌肠整复失败的小儿肠套叠治疗中的应用。方法:气管插管全麻,建立人工气腹,置入腹腔镜探查病变情况并采取相应的手术方式。结果:3例腹腔镜探查发现麻醉后肌肉松弛肠套叠自动复位,仅行肠管固定术;32例腹腔镜下复位成功;8例腹腔镜辅助下腹壁外手法复位成功;2例行肠坏死切除吻合术。结论:腹腔镜手术治疗空气灌肠整复失败的小儿肠套叠是一种安全有效的微创疗法,并可使假阳性患儿避免开腹手术。Objective： To investigate the application of laparoscopic surgery in treating intussusception in which air enema failed. Methods：Forty-five children with refractory intussusception （air enema failed ） received laparoscopic exploration and different operation according to specific situation under general anesthesia in our hospital. Results： The intussusception of 3 cases already reposited after anesthesia and muscle relaxation. Intestine fixation was performed to them. The laparoscopy was successfully performed in 32 cases,8 cases accepted laparoscopic-assisted intestine reposition outside abdominal wall,and 2 cases received partial resection and anastomosis of intestine because of intestinal necrosis. Conclusions： Laparoscopic surgery for children with intussusception in which air enema failed is safe, effective and mini-invasive, and children with false positive intussusception can avoid laparotomy.
