两次成型阻鼾器治疗重度阻塞型睡眠呼吸暂停低通气综合征

摘　　要：目的评价两次成型阻鼾器治疗重度阻塞性睡眠呼吸暂停低通气综合征(OS-AHS)的效果。方法应用两次成型阻鼾器治疗42例经多导睡眠图(PSG)确诊并符合治疗条件的重度OSAHS患者,对比治疗前与治疗6个月后患者自觉临床症状改善情况及PSG检测重要指标变化。结果应用阻鼾器治疗6个月后,患者自觉症状明显改善;PSG检测显示睡眠呼吸暂停低通气指数(AHI)由(66.21±18.64)次/h降到(23.21±11.22)次/h,最低血氧饱和度由(65.66±8.64)%升到(79.12±10.63)%,呼吸暂停最长时间由(89.66±22.32)s降到(46.67±12.33)s,治疗前后各指标比较均有显著性差异(P均<0.01),客观有效率68%。结论两次成型阻鼾器作为重度OSAHS内科保守治疗效果肯定。Objective It is to evaluate the effect of the two forming snoreguard for treatment on severe obstructive sleep apnea hypopnea syndrome （OSAHS）. Methods 42 cases with severe OSAHS by polysomnography （PSG） diagnosis, in line with the conditions of treatment, were treated with two forming snoreguard, compared with the result of treatment between pre-treatment and 6 months later. One was that patients feel clinical symptoms improved, the other was changes of important indicator for detecting in PSG. Results After treated for 6 months, symptoms improved significantly. PSG test showed that sleep apnea-hypopnea index （AHI） low down from （66.21 ± 18.64） times/h to（23.21± 11.22） times/h, minimum oxygen saturation rose up from （65.66 ± 8.64） % to （79.12±10.63） %, the longest time of apnea down from （89.66± 22.32）s to （46.67 ± 12.33） s, there were significant differences （P 〈 0. 001 ）. Objective effective rate was 68 %. Conclusion As a conservative treatment of internal medicine, the effect of two molding snoreguard on severe OSAHS is positive.
