耳廓复合组织游离移植功能性美容修复前鼻孔疤痕挛缩畸形

摘　　要：目的探讨修复前鼻孔疤痕挛缩畸形的方法.方法采用自体耳廓复合组织游离移植的方法对12例外伤后前鼻孔疤痕挛缩畸形病人进行功能性美容修复.术中根据挛缩部位,切口选择在"软三角"或鼻翼外侧脚,做前鼻孔全层切开,松解疤痕;根据鼻翼缺损的的大小,在患者的耳轮上脚切取相应大小的耳廓复合组织,然后将其嵌入移植在创面上.术后观察耳廓复合组织成活情况,通气功能及外形改善情况.结果本组20块移植组织,除1块坏死外全部成活.术后随访所有病人前鼻孔外形和通气功能恢复良好.结论耳廓复合组织游离移植是功能性美容修复前鼻孔疤痕挛缩畸形较好的方法.Objective To introduce a method for restoration of nostril scar contraction. Methods Transplantation of auricle composite tissue was designed in functional and aesthetic restoration of nostril scar contraction. Composite tissue transplantation was used in 12 patients. The most useful donor site was the root of helix. The incision was made in the base of ala or soft triangle. After release the nostril scar contraction, the composite tissues were transplanted in the defect areas. Results The 20 grafts, 19 grafts survived, 1 graft died. Follow-up showed good results in function and apperance. Conclusion Auricle composite tissue transplantation is a good method for restoration of nostril scar contraction.
