摘　　要：目的：探讨电阻抗全血细胞计数对急、慢性白血病的筛查价值。方法：回顾性调查87例经骨髓检查确诊的急、慢性白血病患者首次全血细胞计数的白细胞（WBC）、血红蛋白（Hb）和血小板（PLT）的异常率，观察白细胞分布直方图变化及报警信息。结果：89．3％的急性白血病患者WBC增高（〉10×10^9/L），其中21．3％的患者WBC极度增高（〉100×10^9/L），慢性白血病患者WBC均〉20×10^9/L，其中66．7％的患者WBC极度增高。97．3％的急性白血病患者PLT减少，慢性白血病患者PLT均不减少，两者比较。差异有统计学意义（P〈0．001）。急性白血病贫血的发生率明显比慢性白血病高（分别为96．0％和16．7％，P〈0．001）。急、慢性白血病患者白细胞直方图均有形态改变，R报警率达80．5％，呈单峰分布达85．1％，呈双峰分布为14．9％，均无正常人外周血三峰分布。结论：电阻抗全血细胞计数对急、慢性白血病具有较好的过筛作用。Objective： To appraise the values of complete blood cell count of electric impedance in screening of acute and chronic leukemia. Methods： The parameters of white blood cell （WBC）, hematoglobin （Hb） and platelet （PLT） in patients with acute and chronic leukemia diagnosed by stained film microscopy of bone marrow were study retrospectively and the changes as well as warning information of histogram of white blood cell were observe, respectively. Results：89.3% patients with acute leukemia present with hyperleukocytosis （〉10×10^9/L）, among 21.3% cases were extremely higher （〉100×10^9/L）. All the patients with chronic leukemia present high white blood cell count （〉20×10g/L）, among 66.7% cases were extremely higher. 97.3% patients with acute leukemia had plate-reduction but the platelet of all the patients with chronic leukemia wasn＇t eduction （P〈0.001）. The incidence rate of anaemia of the patients in acute leukemia group was obviously higher than that of the patients in chronic leukemia group （96.0% vs 16.7%, P〈0.001）. The histogram of white blood cell in all the patients with acute and chronic leukemia had changed and 80.5% histogram had R warning. The white blood cell histogram in 85.1% cases showed unimodal distribution, while remaining 14.9% cases indicated bimodal distribution. Conclusion： The complete blood count of electric impedance is a good tool to screen acute and chronic leukemia.
