输精管吻合术后精液参数恢复的研究

摘　　要：为探讨输精管吻合术后精液参数恢复情况,我们对46例接受输精管吻合术者进行术后18个月追踪分析,共检查精液85份。结果提示:输精管吻合术后18个月内,精液中射出的精子总数和精子密度(几何均值)减少;正常形态学的精子减少;精子膜完整性的百分率减少;活动良好的精子减少(a级)和活动不良的精子(c级)增高,这6个异常参数在吻合术后18个月时间内较为恒定。精子总活动率和精子低渗膨胀试验有良好的正相关关系,这两个参数在吻合12个月后有逐步恢复至正常水平的趋势。Objedtive:To investigate the semen parameters at t - 18 months after vasectomy reversal. Methods: Eighty - five semen samples from 46 subjects after vasovasostomy were analyzed using the methods recommended by World Health Organization. The semen parameters were compared with those of 109 normal fertile men. Results: The sperm concentration ( geometric mean) and the total sperm count(geometric mean) in the subjects after vasectomy reversal were significantly lower than that of normal fertile men. They were keptin a constant level in 18 months after vasectomy reversal. The sperm motility at 1 - 6 months was significantly lower than that in normal fertile men. The percentage of category A ( rapid progressive motility) was lower and the percentage of category C(non progressive motility) was higher, compared with those of normal fertile men. The percentage of Hypo - osmotic swelling test (HOST)in subjects was significantly lower than those of normal fertile men and there was a good positive correlation between the results of HOST and sperm motility. Compared with normal fertile men, the percentage of normal sperm morphology was lower, but the percentages of spermatozoa with small head and amorphous head were higher. Conclusion; At 1- 18 months after vasectomy reversal there were lower levels in the sperm concentration and total sperm count, the percentage of normal sperm morphology, HOST, sperm with rapid progressive motility. There was a good positive correlation between the percentage of HOST and sperm motility and they were tending increase after one year of vasovasostomy.
