摘 要:目的研究新生儿缺氧缺血性脑病(HIE)患儿血清中基质金属蛋白酶-9(MMP-9)和基质金属蛋白酶-2(MMP-2)质量浓度的动态变化,探讨其和新生儿缺氧缺血后脑损伤程度之间的关系。方法应用酶联免疫吸附法测定82例HIE患儿发病后1、3、7d血清MMP-9、MMP-2质量浓度,并与17例健康新生儿对照组进行比较。结果①HIE患儿发病后1、3、7d,血清MMP-9及MMP-2质量浓度均高于对照组,差异有统计学意义,两者于第3天均明显升高,MMP-9在第7天有下降趋势,而MMP-2在第7天无明显降低趋势。②轻度、中度、重度HIE患儿血清MMP-9及MMP-2质量浓度,与对照组相比差异有统计学意义(P<0.05);组间两两比较,差异有高度统计学意义(P<0.01),其中重度组表达最高。③轻度、中度、重度HIE患儿发病后第3天血清MMP-9及MMP-2质量浓度,组间两两比较,差异均有高度统计学意义(P均<0.01);重度组表达显著高于轻、中度组。结论MMP-9、MMP-2在HIE的发病过程中起重要的作用,血清MMP-9、MMP-2质量浓度可能是判断病情严重程度及病情进展的辅助指标。Objective To study the changes of serum levels of matrix metalloproteinase-9(MMP-9) and matrix metalloproteinase-2(MMP-2) in newborns with hypoxic-ischemic encephalopathy(HIE) ,and to explore the relationship between levels of MMP-9,MMP-2 and the severity of brain damage. Methods Eighty-two neonates with HIE were enrolled in the study and 17 healthy neonates were studied as control. Blood samples were collected at 1 d,3 d,7 d after onset,and serum levels of MMP-9 and MMP-2 were measured by enzyme linked immunosorbent assay(ELISA) . Results(1) Levels of MMP-9 and MMP-2 were significantly higher in the patient group than those in the control at 1st,3rd,7th days after onset. In the patient group,levels of MMP-9 and MMP-2 increased at 3rd day,and level of MMP-9 significantly decreased without reduction of MMP-2 at 7th day.(2) In comparison with the controls,levels of MMP-9 and MMP-2 in mild,moderate,and severe HIE patients were significantly higher(P 〈 0.05) . The differences between these three groups were also significant(P 〈 0.01) and levels of MMP-9 and MMP-2 were highest in patients with severe HIE(P 〈 0.01) .(3) The difference between these three groups at 3rd day were significant(P 〈 0.01) and levels of MMP-9 and MMP-2 were higher in severe HIE patients than those in mild and moderate groups. Conclusions Serum levels of MMP-9 and MMP-2 increased in the early stage of HIE,suggesting that these two factors could be involved in the pathogenesis of neonatal HIE. MMP-9 and MMP-2 increased with the severity of HIE,suggesting that they might be used as helpful indexes in assessing the severity and predicting the prognosis of neonatal HIE.
