腹型过敏性紫癜12例临床分析及内镜检查

摘　　要：为了促进成人腹型过敏性紫癜的早期诊断,对12例成人腹型过敏性紫癜的临床特点与胃镜表现进行分析。结果显示,12例成人腹型过敏性紫癜均有阵发性腹部绞痛、恶心,呕吐10例,黑便3例。胃镜检查发现12例病人均见十二指肠黏膜散在大小不一的出血斑,3例十二指肠球部溃疡并出血,病理检查见黏膜组织内大量中性粒细胞及淋巴细胞浸润。腹痛发生后1-7 d出现下肢紫癜。表明早期胃镜检查对于过敏性紫癜腹型具有一定诊断价值。To reach the early diagnosis of purpura abdominalis in adult patients,the endoscopic and clinical manifestation of 12 patients with purpura abdominalis were studied.The results showed all the patients complained of paroxysmal abdominal colic,vomiting,3 patients complained of hematochezia and 10 patients complained of nausea.Endoscopy found bleeding spots scattered in duodenal mucosa in all the patients.Duodenal ulcer and hemorrhage were found in 3 patients.Pathology study showed a lot of neutrophils and lymphocytes infiltration in mucosa muscular tissues.After 1～7 days,lower extremity purpuras were found.Early endoscopy is important for the differential diagnosis of purpura abdominalis.
