椎管疾病手术后自体椎板、棘突的复位

摘　　要：目的:对椎管内病变切除后行脊柱重建保持脊柱结构完整,避免椎板、棘突切除后脊柱的变形。方法:对2000年1月～2004年12月41例椎管病变,其中椎管内肿瘤35例,脊髓空洞症5例,血管畸形1例。采用整块切除、术后椎板、棘突复位固定,椎板、棘突切除数1～12个,平均2.8个。结果:经术后1～4年随访,30例完全恢复正常,9例明显改善,2例好转,无1例加重,41例患者脊柱序列均正常,无1例脊柱畸形和椎体滑脱者。结论:椎板、棘突重建术方法简单可靠,可在脊椎脊髓神经外科手术中运用推广。Objective： To preserve the skeletal structure and avoid spinal deformity in patients with intraspinal diseases after operation by laminae and spinousus replacement.replacement To preserve the skeletal structure and avoid spinal deformity.in patients with intraspinal diseases after operation by auto laminae and spinousus replacement;Methods： In fourty one patients, 35 with spinal cord tumors , 5 with syringomyelia , 1 with arteriovenous malformation.The laminae and spinousus of these patients were removed, The number of laminae and spinousus removed 1 to 12, with average of 2.8;the intraspinal lesions were eradicated,then the removed laminae and spinousus were replaced and fixed . Results： In follow-up from one to four years,30 recovered normally, 9 markedly improved, 2 were fair, no one worsened. Postoperative X-ray, CT and MRI examination showed that the spinal alignment were all normal, no spinal deformity and vertebral subluxation were observed. Conclusion： Laminae and spinousus replacement is worthy to be applied for operations of intraspinal diseases.
