摘 要:目的:评价多层螺旋CT甲状腺体积测量的可重复性和一致性。方法:25例颈部螺旋CT扫描患者,扫描数据传输至工作站进行甲状腺体积重建、测量。3位有经验的放射科医生独立进行,每位观察者对所有患者均测量两次。观察者内的差异用配对t检验进行比较,观察者间的差异用重复测量的方差分析比较,相互关系用组内相关系数(intraclass correlation coefficient,ICC)描述,P<0.05为有统计学意义。结果:观察者1两次测得甲状腺平均体积为(12.11±5.42)cm3和(12.04±5.43)cm3.t=1.664,P=0.109,ICC=0.9992,P=0.00。观察者2两次测量的平均体积为(12.09±5.45)cm3和(12.07±5.43)cm3.t=0.445,P=0.661,ICC=0.9991,P=0.00.观察者3两次测量的平均体积为(12.09±5.42)cm3和(12.16±5.46)cm3.t=1.282,P=0.212,ICC=0.9989,P=0.00.3个观察者所测体积的均值分别是(12.07±5.42)cm3、(12.08±5.44)cm3和(12.12±5.44)cm3.F=1.556,P=0.222,ICC=0.9996,P=0.00。结论:螺旋CT甲状腺体积测量具有较高的可重复性和一致性。Objective:To evaluate the reproducibility and agreement of multi-slice spiral computed tomography (CT) in measuring thyroid volume. Methods:Unenhanced spiral CT scan of neck and upper mediastinum was obtained in 25 patients. Thyroid volumes were measured by three observers with 5 years of CT experience of using reconstructed images in CT workstation. Intraobserver variability was calculated using paired-samples t-test and interobserver variability was calculated using analysis of variance of repeated measure. Their agreement was measured using the intraclass correlation coefficient (ICC). Results:The mean thyroid volume in the two temporally separate observations of the same observer 1 was ( 12.11 ±5.42) cm^3 and ( 12.04±5.43 ) cm^3, respectively, t= 1.664,P= 0.109, ICC=0.9992, P=0.00. The mean thyroid volume in observer 2 was (12.09±5.45)cm^3 and (12.07±5.43)cm^3, respectively, t=0.445,P=0.661 ,ICC=0.9991, P=0.00. The mean thyroid volume in observer 3 was(12.09±5.42)cm^3 and(12.16±5.46)cm^3, respectively, t=1.282 ,P=-0.212, ICC=0.9989, P=0.00. The mean thyroid volume from the two temporally separate observations of the three observers was ( 12.07±5.42 )cm^3, ( 12.08±5.44 )cm^3 and (12.12±5.44) cm^3, respectively. F=1.556,P=0.222,ICC=0.9996,P=0.00. Conclusion:The reproducibility and agreement of multi-slice spiral CT in measuring thyroid volume are very high.
