醒脑静治疗急性脑出血的系统评价

摘　　要：目的:评价醒脑静治疗急性脑出血的疗效及安全性。方法:检索Cochrane图书馆、中国生物医学文献光盘数据库(1978-2008年)、中国期刊网全文专题数据库(1979-2008年)、中国科技期刊数据库(1989-2008年)和万方电子期刊数据库(1989-2008年)等文献数据库,收集醒脑静治疗急性脑出血的随机对照研究。对符合纳入标准的临床研究进行Meta分析。结果:共纳入28篇随机对照研究。与基础治疗组相比,加用醒脑静组可明显降低急性脑出血患者治疗期间病死率(P<0.01);在近期疗效上,醒脑静可明显提高急性脑出血患者的临床治疗有效率(P<0.01);治疗前后神经功能缺损评分醒脑静组明显低于基础治疗组(P<0.01);醒脑静组对促进意识恢复、降低体温及治疗前后水肿大小的研究均优于基础治疗组(P<0.01);对促进血肿的吸收和短期内日常生活能力评价的研究和基础治疗组相比无明显差异(P>0.05)。结论:醒脑静可降低脑出血患者的病死率,在改善神经功能缺损、促进意识恢复、降低体温、消除脑水肿等方面也优于对照组,并未见明显不良反应发生,但由于目前的临床研究质量较低,尚需进行严格的、多中心的随机双盲对照研究加以证实。Objective： To assess the efficacy and safety of xingnaojing in the treatment of cerebral haemorrhage. Methods： Databases, including Cochrane Library, Chinese Biomedical Documentary （1978 - 2008）, Chinese Periodical （1979 - 2008）, Chinese Scientific Periodical （1989 - 2008） and Wanfang Electronic Periodical （1989 - 2008）, were searched, and randomized controlled trials were collected. Meta-analysis of the included trials was carried out to evaluate the efficacy of xingnaojing. Results： 28 Randomized controlled trials were included. Xingnaojing was effective in decreasing mortality during the period of treatment compared with basic treatment （P 〈 0.01）. In improving neurologic impairment, xingnaojing was more effective than basic treatment （P 〈 0.01）. Xingnaojing also showed being more efficient in promoting consciousness recovery, reducing body temperature and alleviating hematoma and edema. Conclusion： Xingnaojing is effective in decreasing mortality in short time after cerebral haemorrhage, and it is superior to the basic treatment in improving neurologic impairment, promoting consciousness recovery, and reducing body temperature. Large-scale and well-designed clinical trial is needed to give more persuasive evidence to prove the efficacy of xingnaojing.
