血清p53抗体检测对妊娠滋养细胞疾病诊断的临床价值

摘　　要：目的探讨血清p53抗体检测对妊娠滋养细胞疾病(GTD)的诊断及预后价值。方法2003-2006年西安交通大学医学院第一附属医院,应用酶联免疫吸附法(ELISA)对36例完全性葡萄胎及40例侵蚀性葡萄胎患者治疗及随访过程中的血清p53抗体水平进行检测,对照组为25例健康早孕妇女。结果葡萄胎组血清p53抗体水平显著高于对照组(P<0.01),侵蚀性葡萄胎组血清p53抗体水平显著高于对照组(P<0.01)及葡萄胎组(P<0.01)。葡萄胎恶变组血清p53抗体水平明显高于未恶变组(P<0.01)。血清p53抗体水平与侵蚀性葡萄胎临床分期无关(P>0.05),与预后评分相关(P<0.01)。GTD患者治疗后血清p53抗体呈下降趋势,恶变时再度升高。葡萄胎组血清p53抗体与β-HCG呈正相关(r=0.763,P<0.01)。结论血清p53抗体检测方便、准确,可以早期诊断良、恶性疾病,并可能作为预测葡萄胎恶变及GTD预后评估的参考指标。Objective To explore the value of serum p53 antibody in diagnosis and prognosis of GTD. Methods The serum p53 antibody were detected by enzyme-linked immunosorbent assay （ELISA） in 36 patients with complete hydatidiform mole and 40 patients with invasive mole . 25 first-trimester healthy pregnant women were recruited as control. Resuits Serum p53 antibody was significantly higher in patients with hydatidiform mole than that in controls （P 〈 0. 01 ）. Serum p53 antibody was significantly higher in patients with invasive mole than that in controls（ P 〈 0. 01 ） and patients with hydatidiform mole（P 〈 0. 01 ）. Serum p53 antibody was significantly higher in malignant transforming group than non-malignant transforming group （P 〈0. 01 ）. Serum p53 antibody had no relationship with clinical stages（ P 〉0. 05 ）, but was related to prognosis score （P 〈 0. 01 ）. Serum p53 antibody descended in patients of GTD after therapy. It elevated again when malignant transformation happened. Serum p53 antibody presented as positive correlation with serum β- HCG（r = 0. 763, P 〈 0. 01 ）. Conclusion Detection of serum p53 antibody may be used as an adjuvant method to diagnose GTD exactly, conveniently and economically. It also may be used as a referenced index to predict malignant transformation and evaluate prognosis of GTD.
