摘　　要：目的探讨中央腺体蒂法巨乳缩小成形术治疗乳房下垂及乳房肥大的疗效。方法采用乳晕周围的双环形切口，去除两环之间表皮，形成真皮帽，在皮下沿腺体表面向四周分离达乳房基底部，去除多余腺体，形成能确保乳头乳晕血运的中央腺体带，固定真皮帽进行乳房塑形。结果采用该法治疗轻、中、重度乳房肥大及下垂者共42例，术后形态美观持久，瘢痕轻，无乳头乳晕坏死。33例患者获6～24个月随访，效果满意。结论中央腺体蒂法巨乳缩小成形术适合于轻、中、重度乳房肥大，为瘢痕轻微的巨乳缩小成形术式。Objective To investigate a method of improved reduction mammaplasty with central gland pedicle technique and observe its effect on the correction of mastoptosis and hypermastia. Methods The double circle incision around areola of nipple was adopted and ＂dermal cap＂ was formed by deepithelization between two circles, removal of excessive gland was performed by separating subcutaneously along the surface of the glandular tissue to the base of the breast for forming the central gland pedicle which could ensure enough blood supply for the areola of nipple, and then the ＂dermal cap＂ was fixed for reshaping the breast. Results Forty - two cases with mild, moderate and severe mastoptosis or hypermastia were treated with this method. After 6- 24 months follow - up for 33 eases, all patients were satisfied with the last,beautiful contour of breast, slight scar and without necrosis of areola of nipple. Conclusion Reduetion mammaplasty with central gland pedicle technique is an optimal choice for mild, moderate and severe mastoptosis or hypermastia, which can get good results with slight scar.
