整形外科技术在急诊面部外伤Ⅰ期修复中的应用

摘　　要：目的探讨如何应用整形外科原则和技术治疗急诊面部外伤，使其达到Ⅰ期修复，最大程度地恢复面部的功能和形态，并为Ⅱ期的整形治疗打下良好的基础。方法遵循整形外科原则，根据不同伤情设计不同的修复方法。强调早期彻底清创、解剖复化、缺损修复、无张力分层缝合、应用皮瓣、留置引流管等技术，结果治疗360例患者，无感染、血肿及皮瓣坏死发生。面部功能和外形均得到较好的恢复。其中130例患者术后随访13年，均无明显的增生性瘢痕、组织错位和功能障碍，仅需简单的Ⅱ期整形美容修复。结论用整形外科原则和技术修复面部软组织损伤，能达到功能与外观的完美，减少Ⅱ期修复的难度。Objective To explore how to treat emergency cases of facial trauma, to achieve stage Ⅰ restoration by utilizing principles and teehniques of plastic surgery. Methods The principles of plastic surgery and the right operation methods according to the traumatic condition were adopted to repair the facial trauma through debride in early period, anatomical reposition, repair of defects, suture without tension, application of flaps and place of drainage tube were also performed. Results All 360 cases of facial trauma healed without postoperative infection, haematoma and necrosis of skin flap. Facial function and contour restored well. Among all the 360 cases, 130 cases were followed up for 1 to 3 years. They were all satisfied without obvious hyperplastic scar, tissue malposition and functional impairment, hut some needed simple stage Ⅱ plastic surgery. Conclusion Repairing of emergeney facial soft tissue trauma with plastic surgery methods can obtain perfect function and contour, can decrease the difficulty of the stage Ⅱ plastic surgery.
