摘　　要：目的观察C反应蛋白(CRP)在不稳定型心绞痛患者介入治疗前后的变化,并探讨其临床意义。方法选择不稳定型心绞痛介入治疗(PCI)患者50例(PCI组)及稳定型心绞痛未行PCI患者45例(非PCI组),冠状动脉造影正常患者60例(正常组),用速率散射比浊法测定术前1d、术后2d、术后4周血清CRP并进行比较。结果(1)术前比较:PCI组和非PCI组无差异(P>0.05);PCI组、非PCI组分别与正常组比较有显著差异(P<0.05);(2)非PCI组以及正常组术后2d、术后4周较术前无差异(P>0.05);PCI组术后2d、术后4周较术前均有显著差异(P<0.05)。结论CRP水平升高与冠心病密切相关,阿司匹林、氯吡格雷、他汀类药物联合应用明显减轻炎症反应;介入治疗可以加剧局部炎症反应,但随着血管开通,药物联合应用,炎症反应逐步减弱。Objective To observe the C-reactive protein（CRP）changes in patients with unstable angina pectoris before and after interventional therapy, and to explore the clinical significance of interventional therapy. Methods The interventional treatment of 50 cases of unstable angina pectoris（PCI）（PCI group）and 45 cases of stable angina with no interventional therapy（non-PCI group）,and 60 cases of normal coronary angiography（normal group）. The serum CRP was detected by rate nephelometry one day before operation,two days after operation and four weeks after operation,and was compared. Results （ 1 ）The pre-operative comparison showed no difference between PCI group and non-PCI group （P 〉 0.05 ）, but significant difference between PCI group, non-PCI group and the normal group （P 〈 0.05 ） ; （2）There was no difference in non-PCI group and the normal group two days after operation and four weeks after operation compared with pre-operation （P 〉 0.05 ） but significant difference in PCI group two days after operation and four weeks after operation compared with pre-operation （P 〈 0.05 ）. Conclusion The increased CRP level is closely related to coronary heart disease,and the combined use of aspirin,clopidogrel,and statin drugs can significantly reduce the inflammatory response,and interventional therapy may exacerbate the local inflammatory response,but with the opening of blood vessels and drug combination, the progressive inflammatory response will be gradually diminished.
